Questionable actions that make you go hmm?

Let's have a little fun. What questionable actions have you witnessed by someone that makes you go hmmm...roll your eyes or just flat out laugh?

The pulmonologist who listened to my patients lungs without having stethoscope in his ears.

My last job I went toward the nursing station after being with a patient. The janitor lady was sweeping the counters with the broom. ?

When MDs and staff comes over to the nurse's station, and ask for the nurse not currently sitting there, and if anybody knows why she is not here, and then asks where is she, and looks around. Come on, people, help yourselves and call the nurse.

Peds floor...

Had cold packs that were hard to pop. Nurse put on the floor and stomped on it then was going to put on patient. I stopped that.

Experienced nurse reported at change of shift that phones weren't working for past 1.5-2 hours. Didn't call anyone to get them fixed, House Sup had no idea nor did doctors Didn't call ER who occasionally needed us to start an IV. Didn't call PP who would call us for help with a baby/questions if NICU was busy. So at 1930 call was made to get phone fixed and person had to come in from home.

Experienced nurse comes in 45 minutes late because of snow, evidently didn't think to look out window, nor did his wife. They had previously lived in Alaska! Meanwhile, same night new nurse comes in at 1715 ( shift started at 1900) because she was concerned she wouldn't make it in time because not used to driving in snow.

Saw a nursing student start to walk into a patient's room and start rubbing their hands together and say "I wash my hands". She didn't actually wash her hands, just verbalized it. When told she needed to actually clean her hands, she said that this was how she was taught in simulations.

People walking in and out of a room in maybe 20 seconds, and then documenting the full body exam with the 12 cranial nerves.

When I first started in nursing, the hospital I worked at had Patient Registration type in the admitting diagnosis which would then appear on the Kardex we printed out. There were some pretty interesting dx but my favorite was "Uterine Thyroid" instead of fibroid. That one still tickles me, I can practically see it in my mind haha.

Lol Reminds of the time one doctor (radiologist) saw and talked about a "phallus" in the brain. He meant to say "falx".
