Let's have a little fun. What questionable actions have you witnessed by someone that makes you go hmmm...roll your eyes or just flat out laugh?
The pulmonologist who listened to my patients lungs without having stethoscope in his ears.
My last job I went toward the nursing station after being with a patient. The janitor lady was sweeping the counters with the broom. ?

I see on a regular basis nurses scrub the skin with a CHG prep and spend all this effort to ensure they prepare their sterile supplies just right and hold everything just perfectly to only tear off the finger of their glove and touch the insertion site.

The doctor that does rounds, never taking his hands off the clipboard that he is holding. I'm thinking "you must have xray vision to assess your patient with a bedside chat" ?

Doctors that copy and paste their notes. Or write a note and have never been to the patients room that day.

That nurse who charted PERRL bilaterally on that patient with a glass eye. Or pedal pulses on a patient with bilateral AKAs. Seen both.

And people who chart PERRLA when that’s not what they are testing.
Also, excessive skin parameters. I wish people would stop charting on 10 year old scars or scabs that are from home from a tiny scrape that we are not treating. I literally one time had someone chart on an indentation on the skin. ??

Or that same guy charts "discussed with RN at bedside" and that conversation never happened
My clinic nurse at the VA (I was the patient) documented that I was not suicidal without even asking. Not that I was, but that just isn't right. She had no sense of humor, either. And she didn't wash her hands. ?

I went in to see my PCP after getting strep throat and influenza, that I suspected had turned into pneumonia (it did). The MA checking me in took my vitals and charted RR as 16 (it was at least 24).
At one of the clinical sites for school I watched the cook go into the bathroom with her apron..go pee and didn't wash hands

Then there are the nurses who don't know the difference between neuro checks and neurovascular checks.
