A Narrative Review of VIP Syndrome

Quote

...The term VIP (very important person) syndrome was introduced by Dr Walter Weintraub in 1964.1 Managing VIPs poses a great challenge to health care practitioners. A VIP, by definition, is a person given special privileges in view of his or her status or wealth. Examples of VIPs include royalty, politicians, celebrities, corporate leaders, and wealthy individuals. In my very humble opinion, medical personnel or their relatives have increasingly become VIPs for special treatment.

The care of VIPs is highly varied from one clinic to another because of ethical dilemmas, different perceived benefits for the VIPs, and a vastly heterogeneous group of clinicians and VIPs. VIP medicine is a seemingly insidious phenomenon. Whereas it has been believed implicitly that because of their unique status VIPs are given the best care, faster and greater access, enhanced and more convenient facilities, and special attention from physicians, VIP care can be singularly harmful. The late world-renowned singer Michael Jackson was unquestionably a victim of VIP medicine. He had unlimited access to propofol with the VIP services rendered by his personal physician. His physician acceded to the singer’s demands to receive propofol to help him sleep, which ultimately was a cause of his death. The shocking death of the pop-star Prince opened the door to questions of whether he was an unfortunate victim of VIP syndrome. He succumbed to an accidental overdose of fentanyl. These two tragedies highlight not only the “side effects” of VIP syndrome, but also underscore the complexity of the VIP issue when treating celebrities—for example, demands by the VIPs for potentially life-threatening drugs or procedures, ethical tensions and dilemmas, and breach of heightened privacy and confidentiality when providing evidence in court....

The Challenges In Caring For ‘VIP’ Patients Like President Trump

Quote

..Providing medical care to individuals like Mr. Trump can be a daunting task. Dealing with Very Important People or Patients (VIPs), can pose several challenges. According to experts at Brigham and Women’s Hospital, VIP syndrome in medicine refers to the tendency to treat influential or famous patients differently. In some cases, VIPs may demand preferential treatment for themselves and their families. In other situations, the physician may assume that VIP status warrants special management and privileges. Either way, the clinician often feels pressured to diverge from data-driven protocols just to please or appease the VIP.

“We need to check our responses to the VIP – our emotions, our assumptions – in order to identify them and to keep them apart from our medical thinking,” states Candida Fink, M.D, a psychiatrist in New Rochelle, NY....

...On the surface, it may seem as though a VIP’s social, political and economic influences can “buy” the most expensive, state-of-the-art care. But is it the most effective? According to David Alfrandre, MD, MPH, “The quality of care may be inferior because health care professionals may deviate from standard practices when caring for them.” We saw this happen when the Mr. Trump began taking hydroxychloroquine despite a clear lack of therapeutic benefit against Covid-19. ...

I think figuring out how sick he actually is and his illness timeline is definitely hampered by VIP syndrome because you really can’t get more VIP than the president. Maybe there were good clinical indications for the treatments he’s gotten and maybe they’re just throwing everything at him to prevent a mild case from worsening. I think he looked SOB in the video he tweeted, and his pacing when he spoke was different than his typical cadence. I feel bad for his staff that he’s putting at risk but worse for the people who are suffering because of how this whole thing was managed.

Our cases have been OK here and I actually think our governor has been proactive and reasonably cautious, but it’s still been a huge disruption to my and my family’s life (I have a kindergartner and 2nd grader in distance learning, and that’s been awful for everyone).  I’m thankful I’ve been able to work, for financial reasons but also because my coworkers are my main source of social-emotional support right now and I can imagine how much worse I’d be feeling if I had a job that isolated me more.

I tested positive a couple of months ago for Covid.  I wore surgical masks at work, in public, literally everywhere.  I only went to the store a couple of times a week.  I work a surgical mask from work both places.

My ICU was a Covid ICU in March and April.     I work directly with Covid patients every shift with my N-95.  I never got Covid.

Im not surprised the President tested positive.  He comes in contact with hundreds of people every day.  I don’t believe a fabric mask was going to protect him.

If every American could have an N-95, I think we could tame this virus.  We are medical professionals, we’ve all taken Microbiology, fabric masks don’t work, and surgical masks only help if you have it.  It’s not actually protecting you from the virus.  So me going out in public when I knew I didn’t have the virus, did nothing to protect me from Etsy Debbie and her fashion fabric mask.

Im tired of hearing wear the damn mask and how this is all Trump’s fault and people wishing he would die.  I don’t like the man, but dang, I don’t wish death on anyone.

By the way, I literally had zero respiratory symptoms from Covid.  Headache and fever were the big things for me.  I never once coughed.  I feel very grateful for my mild symptoms.  Nobody in my household ended up with it either, including my husband who I share a bed with.  I isolated from my family immediately.  I kicked my husband out of bed at 2am when my fever started.

I don't know where you are getting your information about cloth masks but I have never read about surgical masks being superior to a 2 layer cotton mask.  My cotton masks fit tighter on my face than surgical masks.

Not talking about N95's.

What really irritates me is that he is the only person in the world getting the triple whammy of medications. He hasn't paid enough taxes to obtain those.

No he hasn't.  But he is the IMPOTUS and he and his incompetent staff have intentionally put the IMPOTUS in harms way and caused him to acquire a deadly virus during a pandemic.  Then that incompetent staff and IMPOTUS exposed and/or infected the entire executive branch and much of the nation's national security leadership. Then they decided that they should lie to the public about how sick the IMPOTUS  actually is while allowing him to jeopardize the health of essential security so that he could behave irresponsibly...again.

We have an obligation to provide the absolute best care available to the highest office in the country because our national security demands it of us...it's appropriate.  The person receiving that care currently doesn't deserve it, but the office does.

Secret Service denounce Trump's parade

Quote

... President Donald Trump has told more than one well-wisher he's desperate to fly the coop.

So on Sunday he did. Just past 5 p.m. ET, a combination of boredom, agitation and yearning for adulation while convalescing from coronavirus brought the President to the rear seat of a black Chevy Suburban, waving frantically as he rode at a crawl down Rockville Pike while two US Secret Service agents, dressed in medical gowns, respirator masks and eye protection, stared stoically ahead.

It was an image concocted by the President himself and the very tight group of aides who have accompanied him to Walter Reed, including chief of staff Mark Meadows and social media adviser Dan Scavino.  They hoped it might reassure Americans that Trump remains both hale and in command....

..But his seconds-long parade only underscored the relaxed attitude toward transmission that appear to have landed Trump in the hospital to begin with..

Afterward, members of the Secret Service voiced escalating concern at what many of the agency's personnel have determined is total disregard for their well-being amid a deadly and highly contagious pandemic. Agents have tested positive for the virus while traveling for the President's political rallies, which he insisted on maintaining even against federal health guidelines...

Shout out to Government staff:

It can take up to 14 days post virus exposure for virus to replicate inside ones body (depends on virility of virus strain).  One or 2 negative tests unreliable. Rapid test Abbotts lab has ~ 25 %  false testing rate.

EVERYONE who was at last Saturdays SCOTUS nomination ceremony or works inside the White House needs to be tested again this week and self isolate to stop this community spread..

Kayleigh McEnany now Covid-19 positive...along with several assistants.
