Did you have to remind me? The last crop of baby docs we got were the worst (so far). I dread going back to work on Tues, and have to explain to them, yet again, (both the first and second year residents because the second years still haven't gotten it) that yes, my DYING patient does need that dose of Diluadid every hour because she is, drum roll please, dying! And that I actually do know what I am talking about when I say that a patient can actually go a whole 6 hours NPO without an IV when their labs are normal and they have had adequate hydration/nutrition the day before (actually the entire admission).

We all start somewhere not knowing a whole lot. I find the vast majority of the 1's eager to learn and the attendings in my facility encourage them to listen to the nurses. I have forged a lot of good working relationships and in some cases friendships with the residents.
I worked last night, so watching them moving around the floors this morning was interesting. My hospital keeps expanding and I gave quite a few directions this morning.

Kudos to your comment. Although July 1st is the first day for new interns/residents with all the accompanying issues, I find the other snarky comment offensive. In a perfect world, the OP wouldn't have to gave anyone directions. Don't feel sorry for you at all.

Thank goodness for order sets!I do feel bad for them but holy moly, I wish they would ask or something before they write some nutty order, no I can't give a unit of blood to my 700g kid, no I can't feed my kid ad lib, he has an
ng and only PO feeds once a day, and for the love of God...read the notes we have all over to only modify the TPN order if a rate change is the only thing changing, now I have to unhide the 20 fluids this kid has had and find it every hour for the next 10 hours to rate validate my IVF. Sigh sigh sigh.

Some baby docs I love, some baby docs I don't. If you answer my page, come see your patient without me having to ask 10 times, and if your answer to my reasonable request at 0200 ISN'T "well let's wait for the day team to get here" - then we will get along fine. I don't mind if baby docs are lost, or put in the wrong order. Just please don't get sassy when I bring it to your attention.
We're all just here to take care of patients, after all.

Oh geez- had this right off the bat last night.
1) Pt wanted to shower- can he come off telemetry to do so? No orders indicating yay/nay, been happily stable the last few shifts. Baby doc punts that to day team. Luckily I caught the fellow and got the go ahead at am rounds.
2) Pt is on tube feeds and D10 because of several past hypoglycemic events, do you want to cut the D10 off and change the insulin order around a bit since that night Lantus last night nosedived her? "Let's ask the day team"
3) Pt admitted for nausea/vomiting. Has one time order for Zofran. That does jack. Page for something else. Also get one time order for Phenergan. Hello, order sets! Other nurse had to explain the concept of PRN meds.
