This morning, which is my second last day to finishing up my 2nd semester clinical, one of my residents was due for a glycerine suppository. Unfortunately for me, I didn't double check one and Gravol suppository was given.
My instructor gave me hell for the first couple hours but calmed down...after things were settled with the Family doctor and the family of the Resident. Now, the school campus was notified and told my instructor that it's her decision whether or not I get to pass/fail.
She really doesn't want to fail me..because this is her first time instructing for clinicals and she just wants to be on good terms with the school and the group.
I felt so bad..throughout the whole day.. We had a long post conference talk about this matter and I was so upset at myself for doing what I did. She said she'll give me her decision by tomorrow..
I really don't want to repeat because it was 6 weeks of hard work (I was responsible for 5 residents) and I had one of the hardest clinical instructors out of my whole LPN class.

I hope she gives you a chance... any med error is serious, but given the circunstances...
I rather do something wrong at clinicals, then when I become a nurse...
I am a braniac when it comes to check my medicine and i will tell you why I almost failed my last semester of clinicals:
1- good morning nurse XXX, my name is YYY and I am your nursing student for the day, I will be taking care of these patients, I will provide complete care and meds for them, other then IV PUSH DRUGS. OK??? so then i will see you after my preconference!
2- (after preconference) Hello there Nurse XXXX, so I will begin care for my patient now, remember that i will do all the meds, care... ETC... BUT IV PUSH!
3- here I come, check on the MAR, its time for insulin, WHICH SHOWS NOT DONE read the glucose check, sliding scale asks for 2 units (PT NPO, convert scale to 3 units per DR) get the insulin, do all my checks with my instructor and another 2 RN's, go to the PT room, (whom can't talk and is not really "with it") give him the insulin, get outside the room to chart... till there no problem
4- here comes the nurse, flying in to the room and ask what were I doing, so then I said.... giving him his insulin (the RN was not a regular at that floor she was training from a SNU) so this lady turns PALE white, and say, i just gave him insulin!!!
5- i get the looks from my instructor, i get red as a volcano, and say: why did you do that ? I told you i was doing... bla bla bla...
6- she blank stares at me and says... i forgot, i gave it 10 minutes ago!!! (I DO NOT KNOW WHY SHE DID IT) she had 3 pt load, which were all taken care by us other then 1 of them.
7- I right then and there told her in front of my instructor (very politely, but scared and mad!) that I had informed her twice (which she agreed to, but "forgot") and told her, that she had not documented so, if she had documented, i would not have done it!
8- THANK GOODNESS my instructor understood that it was not my fault and also that it was only 4 units total (and the guy was getting out of NPO at that same hour, so his need for insulin would be more)
9- we followed procedures, and everything turned out ok!
*** now, I learned from it, that nurses can get busy, and forget what we tell them, so after that, I now bring my instructor right after getting report, and reasure that the nurse is aware.
I also do a quick check and inform my nurse of what i am doing (like a tweeter live feed LOL)
and i check, my order, write them down on my notes, then prior to administration, i recheck the PC and the real chart just to make sure once more, right then and there....!!!

WOW I would surely hope their would have been no way for you to get in trouble in this case, I don't see what you could have done different. Thankfully all turned out ok.
even though I felt like kicking the living s.... out of her, i understand that she was training, and forgot...
I am getting very anxious about my whole ordeal.
I just wish I could reverse time.... WHY does this always happen to me? 2 more days and I blow it......
i dont know what to do.

deep breath.......deep breath......one of the biggest things is realizing that the patient did not suffer harm. that's the biggest.
if you think that's bad, one of my classmates gave dig to a dead patient. top that.

Anyways... your classmate gave digoxin to a DEAD patient? What?!?! lol

haha..i too screwed up in my clinicals i almost injected a next bed patient with an infected syringe coz the patient was an ISS.i dint get to tell anyone bt i was so afraid.i just smiled and walked away.

Try not to freak out, and just wait to hear from your instructor. If you've done well the rest of the semester, you'll most likely just get a warning. Good luck, we'll cross our fingers for you!!!
