My handoff to the oncoming rn was "300 pounds, off meds, homicidal intent". The was more of course but that summed it up. Let's hear 'em.

The best reports are just facial expressions because we both know this patient and we both the story. If you come to the er every day by ems, no report is needed.

In my first semester was going to LTC for a day. Our instructor did warn us that we would be surprised at how different it is here compared to the hospital, especially report. One nice nurse decided to give report to all of us. She gave individual report to two students and when she saw us all lined up, she said "Well, everyone down this hall, so rooms 19 to 32, are all doing fine."

work peds LTC, we give a more detailed report at shift change, but for break coverage this is the standard we give

On a PVS pt who had been at the rehab/LTC for 10+ years:
"She's fine. Mom filled the candy dish!"
