My handoff to the oncoming rn was "300 pounds, off meds, homicidal intent". The was more of course but that summed it up. Let's hear 'em.
"Look at me how do you think it went! My hair was straight, combed, my clothes were clean, oh and I I'm glad I did stop to urinate BEFORE I came here. They're sleep, fed, breathing and pain free have fun! " counts cart and walks off. Calls back " oh I typed report ....sorry it was crazy today" 12 hours from the 3rd realm of hell is all I can say luckily I did type report and my coworker was my regular.
My first day working in LTC, i received report "Betty, nothing new. Sarah, she's ok. The boys is 212, well that one had a high sugar but the other is ok." I was seriously in tears after the 20 patient report. I was used to M/S reports with name, age, dx, surgeon, heart rhythm,last narcs, dressings/drains/incisions/mother's neighbor's daughter-in-law's underwear size.

I work hospice and the best one sentence report I have ever received: Increased dilaudid to 250mg per hour, versed to 20 mg per hour, and she is still alert.
Also: pt is seeing her cat that died 15 years ago, so reviewed dying process with family.
