That It's a pommel cushion, not pummel cushion. A pommel is the part of a saddle that prevents a cowboy from sliding forward, while pummel means to beat something repeatedly, usually with ones fists. I'm not beating a chair cushion, no matter how mad I am at it. :laugh:

Please enter an order for a MAT to the floor to prevent injury, not a MATT to the floor. My friend Matt is not going to lay next to the resident's bed and catch them if they fall. :roflmao:

It's STRAIGHT cath, not STR8 cath. It's a medical record, not a Grinder profile :cool:

When charting in a computer, and you are trying to chart a stage 2 pressure ulcer, please don't chart a "stage 11." If you're wanting to use roman numerals, try "stage II."

Butt crack VS natal cleft.

I've never seen or even heard natal cleft. I tend to use gluteal fold.

actually it is gluteal cleft, gluteal fold(s) are the horizontal areas at the bottom of the buttocks/top of thighs. never having heard of "natal cleft", i looked it up, that is appropriate usage.

This one reminds me... Pt states he developed diabetes after being exposed to "Asian Orange" in Vietnam.

ETA: this was the RN's documentation. The pt verbally told me it was "agent."

Just to prove to some of you that I can be light hearted.....

I had a nurse who charted on the dementia unit: resident has cognitive defecate. I asked her if she meant #*&% for brains.

She was a wonderful nurse...she just couldn't spell.

I had gotten report on a resident dx of UTI coming back from hospital. The nurse said "His urine has some floaties in it"

"specimen cup given to caregivers so they can collect dementia."

Worked with a nurse that always used "panties" to describe a patients underwear in her charting. "The patient needed a clean pair of underpanties" even when documenting on a male patient!

another one that bugs me.. "Prostrate", umm there's no "r" after the "t"!

There are a number of residents in my facility wearing tet hose.

Yeppers, worked with an LPN who INSISTED it was O2 STATS. INSISTED. Little wonder that 5 years after graduation, and at least that many attempts, she's yet to pass NCLEX-RN.

The activities director would do monthly charting on residents. Almost all of them were "independented" and made their own decisions regarding the activities they participated in. Just try to SAY independented!
