I'm not sure which story you're reading, but every news source that I've checked (online articles and TV interviews) make it sound like she is dead serious. She has stated that she hopes that this will be a wake up call for Western State Hospital, getting them to change their ways.
True, there are a few problems at WSH that they are trying to work out, but it seems like Sara has decided to be a part of the problem instead of the solution.

I read in a couple places she wanted a last laugh with her coworkers. And then in the KOMO news peice
"She resigned last week after working at the hospital for seven months, but sent the cake to her co-workers as goodbye, KOMO-TV reported."
As someone who's been misquoted on more than one occasion by news mediums I can easily see how and why the "news" would try and make her out to be a disgruntled nut. She actually is trying (in real life) to bring awareness to the lack of funding for mental health.

I totally agree that the lack of mental healthcare funding is a major problem. However, I'm not buying it that she is a totally innocent, misunderstood crusader. While amusing, the cake is, at best, immature and unprofessional. No matter how hard she fights for the mentally ill (and I hope she is able to make some positive changes), I cannot overlook the silliness of this action...sorry.

I looked up the story, and found this; she's not shy about badmouthing her former employer to the press:
