The RN I was training with on clinicals as a 2nd year student did not just tell the patient that I was giving meds to that her Simvastatin tablet was for her reflux.... YES she did I had already checked it against the medical order but after she said that I checked again incase as the student I'd mucked up, pointed to it and asked her 'this one', yep, ummm actually no its a hypolipidemic med. I don't like to correct anyone in front of the patient but heck even the patient was looking bewildered about her new reflux medication!!!!
Later the same nurse looked at my careplan and asked why I was going to take a post op patients obs every 4 hours, I got his carepath and showed her where it was written that Vitals were to be taken on day 2 as 4/24... Meaning four hourly, she said NO, 4/24 means QID I asked her what would four hourly be then and she said 6/24 :loveya: God love her. But she made me change my careplan to QID Obs on this patient.
no my instructor did not just tell us that PICC line stood for "percutaneous intravenous catheter". she was even missing a C!
no she did not ask her instructor what this strange port in her patients chest was, referring to one of the telemetry stickers... and we were on the telemetry unit.. lol.. that was me..
no she did not nervously tell the patients husband that this was the first time ever discontinuing an IV on a real person. - me again. ooops.
no our instructer did not tell us not to listen to our coassigned RN because she used to be a teacher at our school 2 years ago, and was fired after 6 weeks!
-No she didn't just tell that young blood clot prone patient living in a half way house that she should keep her embryo instead of having an abortion, because "lots of people would like to have babies"
-No she didn't just ask the patient the correct medical terminology for her condition, because she was unsure.
Haha I'm not even in nursing school yet, these are just things I've witnessed from nurses in the past few years.

NO SHE DIDN'T go into the wrong room and assess the wrong patient all day and never realized it until post conference......thank god she didnt pass meds... Can we say, " Can you please verify your name and birthdate for me." LOLOLOLOLOLOl!!!!
