How do you sign your name?

This is inspired by another thread. How do you sign your name when you sign off on stuff at work? I use my first initial and full last name. Like this: O. Bobcat, RN I have a pretty unique last name, and there are no other nurses in my state with my first initial and last name.

My boss at my previous place of employment told me I needed to use my full name when I signed, but I never changed the way I sign after she told me that (I'm such a rebel!)

I use my full name including middle initial when I sign non-nursing related things.

I rarely sign anything because all the documentation I do is electronic. Sometimes I do First Name Last Name, RN, sometimes K Last Name, RN. I usually sign admission agreements and insurance appeals with my full name and lab reqs with my first initial last name.

And funny enough I just had to reprint and sign a note to my daughters school b/c I kept putting CT Pixie, RN. ughhh!

More than once, I've signed school permission slips, credit cards, my taxes, the consent/release for my tattoos, etc., as M. Meriwhen. RN-BC. Habit :)

A(garble) C(garble), RN. My kiddo laughs at me every time I sign a credit card slip and tack the RN on the end. I do EMR now so I don't sign as much as I did at the paper charting job, but once you do it for a while, it kinda sticks. :)

I don't write checks anymore, but when I did, I tried to remember to sign Ruby V. Vee -- usually came out R(illegible scribble) V. V(striaght horizontal line). (Thank goodness for online bill pay!)

I use my first initial and full last name followed by RN. I don't use script. I sign in print - if that makes sense lol.

I still "sign" things even in my electronic chart. AChick, RN. That's what we were taught to do.

Old job: first initial and last name. New/current job FULL NAME! First, middle, and last name along with credentials.

first initial surname all in cursive then my role and in brackets my surname printed afterwards.

Unless it's for TAFE in which case I have a scanned "signature" on my computer I can insert in, it's basically the same but at a different angle and smiley face doodle on there as well.

And like so many others in this thread, this is also how I usually sign permission notes for my son's school, banking paperwork, etc. Even when I consciously think, "This is not for work, don't add the RN at the end" as I pick up the pen, nine times out of ten that darned "RN" still ends up on the paperwork after my name. :wacky:

I go with first initial, last initial follow by a wavy line. I figure if docs can sign prescriptions totally illegibly, I can sign discharge papers illegibly.
