12/19 What I Learned This Week: night shift with Macaulay and polio

The thing that has me the most wound up this week? Polio. I may be the last idiot to learn this, but our CIA used a vaccination campaign in Pakistan to collect DNA from Pakistani children, trying to find the children of bin Laden. Vaccination rates there have plummeted as a result, making a nearly eradicated disease remain a current threat.

I've been totally over this Christmas since last Christmas.

It sucks when you have an elderly, full blown dementia patient who is both violent and a screamer, and your hands are tied to say what's going on when one of your other patients is deeply bothered by it, saying, "why can't they just leave that poor man alone?"

A man who wants to die will not tell you when his chest pain is 10/10. He's only there because his daughters made him come.

Never remove an African American woman's wig.

Even nurses might not realize ischemic strokes don't kill you. Skipping anticoagulants might have worse outcomes than death.

Ensuring long blocks of days off on a regular basis seriously is crucial to recharging batteries.

I am still very much baffled by the fact that some respiratory therapists choose to smoke. Nurses, too. Look, I love you all, but if you've worked on a unit that gets bipap dependent or vented patients, surely you have some inspiration to quit? I'm not judging, as a former smoker myself, and I know how hard it is to stop. I just don't understand.

I really hate having to adjust to being awake in the AM after working nights, and I am so incredibly glad my husband is switching his hours to daylight so I don't HAVE to adjust my hours by much.

Hospitals are being fined millions because multiple patients were admitted for what Medicare feels should have been outpatient procedures. I'm annoyed by this. Perhaps they didn't require admission. Or, perhaps they were high risk for complications. I really don't know and wouldn't fault a legitimately applied fine. The problem is these massive sweeping fines make our MDs hesitate when they shouldn't.

Obama is a lefty.

The problem with being open minded is struggling to have actual opinions.

In spite of the night shift hangover, it feels so good to be back on night shift.

I've learned that parents hate when you send their little one home with pink eye for 3rd time in 5 weeks.

I've learned that the same parents did not realize that when you share eye drops, pink eye spreads!

4th grade girls are unbelievably petty.

Even kindergarteners fake sick to get out of gym class.

I've also learned that my 5 y/o-almost 6, mom!-thinks that tooth fairly is ridiculous and the easter bunny is absurd, but we are totally cool with Santa and the elf on the shelf!

I've learned that being off for the holidays is incredible.

I am a HUMAN again! I finished my fall semester with a 4.0 baby! So I'll be bothering everyone on Allnurses a lot again.

I realized I only have 4 weeks until this baby is due, I have a lot to catch up on before he is here!

I found out my part time employer cut our PTO hours so I'm not sure what we are going to do $$$ wise as I need at least a few weeks to recover/take maternity leave. All I can say is thankfully I purchased my own short term disability insurance, and my FT employer offers a plan that covers my benefits during my leave.

That the newer "fancier" hospital really is not the best when it comes to care, so I'll be going to the less flashy, but better care providing hospital with this baby as well.

That my HR dept is more dysfunctional than ever, despite this new "magical" re-organizing that was going to make rainbows and unicorns appear.

That my new CFO is as sneaky and shady as I had thought.

I was reminded that some of my co-workers are the best friends I could ever ask for, and that some of the MD's I work with are beyond kind.

That there is a yellow fever vaccine shortage in my area and there is a local mission who is taking a lot of kids on a trip to Africa.

It's probably one of my favorite scenes of all time from a comedy.

I have just learned that if you allow your girl child to hang all of the "special and important" ornaments in "the middle", the tree may be heavy on one side, and it may fall down. So glad the special stuff was mostly glass.

While almost all of the best lines violate the tos, i did also really liked the sight gag from the wedding set up by the flash.

I learned that seeing 11 patients in one day (between the short week and having to cover for a sick co-worker), even though they are all in two facilities, really really blows.

Really.

Especially when there are fires to put out along the way.

I learned that I like working with BPD patients more than the ones who really need to be hospitalized.

The best thing a patient said to me this week was "you don't like me". I answered, "I don't need to like it when you curse at me, but I will continue to do my job. And my job includes not giving you the Ativan. I don't get to take the Ativan that I don't give"
