Sorry to steal the same title as the BP one. Patient was in the ER today, was reviewing home meds, and she was on
130mg Oxycontin Q TID (not 3 divided doses) and 30 mg Oxycodone Q TID I was shocked, the ER nurse I was with has been a nurse for 22 years and she had never seen a dose this high either for someones personal home meds.
Just curious what others have seen.
My husband is on Oxycontin 160mg q 6 with IR 20mg up to 3 times a day. he has failed back surgery syndrome and 2 knees awaiting replacement. He was injured on the job (L5 & S1 chipped and pieces ended up in the nerve root) in 1994. I watch him cry in pain when the weather gets cold and damp here in NY. Yesterday was 1 of those bad days. But we are married 14 years this past Sept and we make each other laugh at least once a day!! And I love him with all my heart!
I just wish he wasn't in so much pain all the time.
Nothing stands out for me re oxycontin, but:
-pt w/ terminal ca, 550mcg fentanyl patch + dilaudid for breakthrough
-pt w/ end-stage HIV disease on MSO4 @ 750mg/hr
-pt w/ terminal ca in pain crisis w/ dilaudid @ 15mg/hr and 15mg q30 min for btp, - had over 800 mg in 12 hr. Turned out he was experiencing opioid -related hyperalgesia AND benzo withdrawal.
I can't remember for oxy, either. but I had a 35 yo man with severe spinal problems (I forget the details) on 700mcg of Fentynal in patches, as well as a PCA of up to 200mcg of fentanyl on top of that. he was awake, alert, speaking.
Then I had a dying cancer patient on 14 mg an hour Dilaudid (continuous automatic PCA drip) as will as 1 mg ativan every hour iv push and a supplement of 4 mg diluded iv push per hour PRN.

I may have you all beat. LOL... I have a good friend that has chronic pain, Degenerative Disc Disease, has had a fusion and failed back surgery and the list goes on. Anyway, she has been taking oxycontin for almost 10 years now. After her fusion she was actually on 280mg q8h. Of course, she began on the 10 mg tabs and worked up and up and up. The good news is that currently she has been weaning herself down for a while now and she is now on 120mg A DAY!!.. She takes 80mg in the am and 40mg at bedtime. Of course she is still in a lot of pain but after being on this drug for so long it began to work against her. She has an appt. with a new pain dr. on the 9th of this month and I am praying he can help her out. She is pretty much bed-ridden. So sad. Chronic Pain is just horrible.
