At my school they are called Process Recordings but I think they're the same thing. What you have to do is turn in a written record of a therapeutic communication you had with a patient, and your record has to demonstrate:
1. Priority Nursing diagnosis for this patient
2. demonstration of working through each phase of the therapeutic communication (pre-orientation, orientation, working, termination)
3. demonstrate use of a variety therapeutic comm techniques (restating, reflection, empathy, paraphrasing, etc.)
It is written in the form of a dialogue between yourself and the patient, in which you help identify a problem/concern with the patient and help the patient draw on personal strengths to problem solve. For example, the last one I turned in was on a pt. with anxiety. So I used therapeutic techniques to help him problem solve and identify healthy ways of coping with his anxiety.

Thanks for posting! That was great!!! :wink2: My favorite parts were the students telling about what specialty they wanted to be in and why they were getting into nursing. Awesome!
Mixy, thanks for clarifying about process records. I was wondering about that too.
