The worst or strangest orders you have seen...

This may have been done before but I would like to hear about some off the wall orders you've seen MD's write...

Here are a couple from me...

Give 2mg Coumadin qd and prn :uhoh21:

Zantac 150mg q6hours and prn

One of my favorites:

"May insert foley if patient so desires"

I know I've seen some others, but it's been awhile since I've seen some good ones. Probably because we so rarely get off-service patients on our unit, and the only residents we get have at least a couple years under their belt. Our orders are usually pretty standard.

This one is an "oh-yuck" for me.

Had a pt once that absolutely LOVED a pretty nurse to come in and insert the ol' foley - He said he just felt he could pee better through a rubber tube....

Referred for psych consult - me, not the pt.

This weekend we rushed a patient to ICU. We coded the lady and had fluids wide open and Levophed cranking for a BP of 50/D. The new resident ordered Lasix 40mg IVP because the patient had no urine (No kidding! That tends to happen with a BP in the toilet!). :banghead: Then they wanted the IV fluids to run at 125ml/hr. Um........."Again, BP is 50 over doppler, we have fluids wide open and Levophed at 20mcgs, how about we add another pressor instead?"

Funny how the resident that was clueless in June is now the senior over the new ones in July. :uhoh3: :banghead:

I've seen this on a psych unit that you aren't served caffenated drinks without a dr's order for soda or coffee.

I was a new nursewhen one of the older nurses received an order to ambulate her surgical patient QID. I was standing at the station when she called the surgeon to tell him that he was going to have to come help ambulate the patient - he was a paraplegic!

She asked the surgeon if they were supposed to take the guy out for a drag.:lol2:

I know it's not really funny, and the surgeon called her a name, but we all laughed at him!

these dr jus happily order treatment wifout thinking!

when i was a student, i'd a patient who's abt 100+kg wif severe contracture and stage 4 sacral sore. guess what was ordered?

"SN to kindly sit patient out of bed tds"

so, my senior nurse replied "would appreciate it if team drs could advise and assist in transfer in view of multiple dripping sacral wounds";)

For a 500# pt: Arrange for transport to SeaWorld for MRI...SeaWorld declined, stating they would not examine a human.

For a pt that tried to disempact her bowel with the fork from her dinner tray (perfed herself, too): Only plastic spoons on meal trays from now on.
