This may have been done before but I would like to hear about some off the wall orders you've seen MD's write...
Here are a couple from me...
Give 2mg Coumadin qd and prn :uhoh21:
Zantac 150mg q6hours and prn

Turn pt q2h. ( Duh I really needed that order thanks Doc!, probably written by new resident since it is July)
Salt tabs 2gm po q4h prn hold if NA >140 ( I didn't have time to question this one , they pt was hyponatremic and on 2% sodium drip)

You know what I hate...
When a doctor orders new meds and doesn't let you know what to discontinue or what to keep on the old med orders. They get paid the big bucks and we have to keep them straight!
Here is another one I had last Thursday night... this one is good and I forgot it initially.
Clindamycin 600mg IV q12h po x7 days. I called to clarify whether he wanted it IV or PO but was thinking, ok... do you want it IV or PO OR just squirt the IV in the mouth?
Our medical director is a QUACK!
I still love the coumadin PRN order!

I know I've seen some others, but it's been awhile since I've seen some good ones. Probably because we so rarely get off-service patients on our unit, and the only residents we get have at least a couple years under their belt. Our orders are usually pretty standard.

I had to chuckle on this one. I have not seen a patient desire a foley...

This was written by a very tired, but competant ER doc....we both had a good laugh about it when I pointed it out to him. (It was on a discharge Rx).
Vicodin 1 gram IV Q4-6 hours as needed for pain

Not really sure what he was thinking.....but he was thankful I caught it...we all write stupid stuff. Today near the end of a very busy shift, I was charting off on a patient and the time I wrote was 1899. I guess that means one minute before 1900, right????
