The worst or strangest orders you have seen...

This may have been done before but I would like to hear about some off the wall orders you've seen MD's write...

Here are a couple from me...

Give 2mg Coumadin qd and prn :uhoh21:

Zantac 150mg q6hours and prn

I have more too but need to think:lol2:

you know I think these orders from doctors should be lined up in ... the television series of......BELIEVE IT OR NOT.

I remember calling a resident about a C-Section pt. who was complaining of a rash from her abdomen to her thighs. He came in and looked at it, and wrote some orders, including "Give Dilaudid 2 mg q6 hr around the crotch".

What was he thinking? He meant "around the clock".:lol2: He never lived that one down, we still laugh about it!

Yeah I work in the NICU and we give caffeine citrate IV all the time for apnea of prematurity.

Thanks! I know now... a couple of people have responded to this. I had no idea about IV caffeine but it all makes sense now. I have never worked around little babies.

We have caffeine tabs at my work for headaches.

I had a patient who was in for a headache... mind you she was reading and watching TV all hours of the night. and had an IV running... She wanted the foley just so she wouldn't have to get up. I told her no, she was very capable of going to the bathroom, that I wasn't even gonna ask the doc for it. She didn't push the issue :bugeyes:

One of my favorites:

"May insert foley if patient so desires"

I know I've seen some others, but it's been awhile since I've seen some good ones. Probably because we so rarely get off-service patients on our unit, and the only residents we get have at least a couple years under their belt. Our orders are usually pretty standard.

Why so weird? After my baby I couldn't urinate - so the Doctor order a foley to be reinserted if I CHOSE because in case I:trout: was highly uncomfortable and couldn't void - BTW, the nurses did NOTHING to help me void! Now I know I'm a nurse, but is that a reason to get substandard care?

OK - I have one. A patient came in with scalp wound (and I mean practically SCALPED!!!). His B/P was dropping and the doc quit suturing and went to the vending machines and got a Sprite and wanted one of the nurses to re-hydrate him with Sprite (I know, I know - 7-Up is the DOC for rehydrating! Ha! Ha!)))))

Well, we called the ER Head Doctor and got this guy out of our ER - He NEVER came back to work another shift again!. BTW - this wasn't the only wacky order he gave, just one of the highlights! :monkeydance:

hey my Dad got beer prn in the Nursing Home - he was dying of liver failure and such - NOT a candidate for transplant - refused dialysis - wanted to die as he lived - DRUNK!

I had the same problem after 2 of my deliveries. (I have 4 kids) I had urinary retention and had to go home with a foley. I basically had to retrain my bladder. After I had my first daughter and the nurses thought I was nuts because I said I couldnt void. They kept saying "Yes you can... give it time" Well they straight cath'd me for 1000 ml's 6 hours after delivery! Boy was I uncomfortable.

My last baby (she just turned a year old) was delivered by emergency c-section... I didnt have urinary retention.

I have 4 kids -- my older is almost 17 and I didnt have any bladder issues. My middle son is 8 and I had terrible bladder problems. My daughter is 3 and I had the same bladder issues (couldnt go) and this went on for about 3-4 WEEKS after delivery. I had to go home with a foley and have a home nurse come to me... etc, etc.

Then my last daughter--no problems. I guess it was the trauma of labor and delivery????

Sorry to ramble (and I went WAY off topic)... I just haven't talked to anyone that had the same issues. And I guess you are right about wanting a foley because I DID want to be cath'd after 6 hours of NOTHING! I was hurting!!

We get that a lot with our hospice pts...if they want a foley, they can have it.

yeah, but the thing was, this was a med surge patient with no mobility issues and no retention issues.

Just an early July admission with a brand new intern writing the orders.

I had a doctor write an order to shut off the lights in the room. Hi walked right by the switch. He was a resident and after a little discussion I hope he never writes another order like that
