The worst or strangest orders you have seen...

This may have been done before but I would like to hear about some off the wall orders you've seen MD's write...

Here are a couple from me...

Give 2mg Coumadin qd and prn :uhoh21:

Zantac 150mg q6hours and prn

Just got an order last night on my icu transfer to please provide toothbrush and toothpaste. lol!!

Wish my doc would have written that order when I was in the hospital. Mine kept growing feet! I am not kidding! My nurse was like where are they going? I went through 3 toothbrush and paste kits! Still a mystery!

immaterial really mouth care is a part of good nursing care regardless of where or what the patient is /has (unless their only complaint is Pajama Induced Paralysis when they can damn well clean their own teeth)

All of our suppos that come from the pharmacy are labeled like this, always thought it was strange myself but someone, somewhere along the way must have ****** up or it wouldn't be written that way.

"Valium 5mg po now, give to patient's wife for anxiety."

This past summer my uncle went through a prostatectomy-sp?- and the docs were more concerned about my aunt and her anxiety and panic, it was giving her diarrhea, anorexia and hyperventilation, that he did write Rx for her to get valium.

At a hospital where I worked previously, when a patient died, there was a spot in documentation for how we had handled family reaction to the death. And there was a spot for charting meds given to family member that the doc felt needed it. I never saw anyone get anything but it was there.

Once in awhile when a peds patient dies, the doctors will order stuff like that for the parents. I've also seen sleeping meds ordered for family members.

My 13 year old son was killed last year. Not only did the dr order meds, based on a phone call from a friend of mine, the pharmacy delivered them STAT and when I asked how much my co pay was the pharm said don't worry bout it.

Indeed. My wife (also a nurse) was an Army medic, and she says that if there's a warning like that on something, someone screwed up somewhere. She says that Claymore mines carried the following warning on their packaging: "This end towards enemy. Not a food product." Hmmmm....

I believe that warning was put there because some soldiers during a certain time period reportedly ate the explosives inside to avoid going on patrol. Hospital was considered better than the jungle.

My Dr. ordered some ativan for me when my dad died suddenly. I was losing it, and I was glad I had it to get some sleep, even though I certainly never asked him for it. Gave some to my mom too. She never took it, but should have.

My order: a few days ago I got a call from "Dr. Evil" who began screaming at me about "Do NOT ambulate that pt!". The same one he had written "PT to ambulate" orders on the same morning. Apparently pt's family had called Dr. to complain that therapy was in there! :icon_roll
