I recently took a unit manager job ina SNF. My clinical experience is minimal, common sense present.
Today I had a fall for which the LPN had to call me for the RN assessment. Heh. I stood there as she did everything and then I helped lift the resident from the floor with the blanket roll she knew how to make. Amazing knowledge. I signed the assessment and called for permission to ship her to the ED for xrays and a CT. Hit her head hard. Pupil sluggish on the side opposite the impact. Another LPN had done the neuros.
Then I'm back on my own unit. A resident says her chest hurts right in the heartburn spot so we give her a Tums. I want off to a meeting and come back to see the local rescue squaf coming in to ship her to the ED. She was admitted to tele.
Once again, you experienced, smart, knowledgeable nurses did it. And once again, thank you. I sure couldn't do my job without you.

Hey, I was one. And I don't know where they're being phased out but we sure need them here! And a good thing, too. Our new-grad BSN straight into a managerial spot really thinks she is a superior nurse because she can care plan. That's great, but when my old one falls down goes boom I want someone who knows what the hey to do, and it's the LPNs with 20 years on the floor, not her and at this point it is starting to be me but I would be lost without these years of clinically-astute women to support me. Care plan this!

Reminds me of when I was working an agency shift at a nursing home; one of the resident care managers who had her MSN but little clinical experience (VERY nice woman) came booking down the hall saying so-and-so needed to be sent out 911 b/c he was unresponsive. I said "What's his blood sugar" and got kind of a blank look. I went to the dining room, checked his BS, it was in the 50's; so I gave him some OJ and he came around. I felt really bad for the RCM, she looked like SHE was the one who needed sent out after that happened!
It's always good to be reminded that we're all on the same team. :)

I am hopefully starting a new job as an RN in LTC next week. It will be my first job as an RN and I will definitely be relying on my seasoned LPNS and CNAS so show me the ropes. I may have RN behind my name but they are the ones that know what's up.
