SHould I move?

Never been so lost..... I have had the chance to work with fantastic colleegue in the best environnement possible for me to grow as an individual and a nurse. I have always showed interest into nephrology and must say that for me, it is a must!!! And after 12 years in this environment, and many acheivements, I have to decide if I continue where I am or if I move towards a new carreer. I have the chance to work in a Nephro clinic. Alright, I know I should be trilled but it means mon-fri, 8hrs ( and a few more hrs work) and a bit of loss in salary since you have to count for the premium that you dont get anymore for not working evenings and w/e... And not to mention, that I wont perform as many techniques and therefore will loose some of that knowledge and dexterity I love..In a way it is the job I have been dreaming of but I wish it would be 10 or 12hrs/day so i would have less days. I wish I could keep up the knowledge I have and be close to my team. When you have not moved for long, it becomes difficult because your roots are stronger and dont want to let you go. And even if I know my job would be interesting is that the right one.... welll I guess I am just as confuse and could use a few comments or suggestions.

all I can tell you is the generic response...but it does help to evaluate the situation...

Writ out lists of the pros and cons of each situation...then go back a couple days later and see if they're the same...then make your decision.

I left the hospital 11 years ago and went into home health then LTC. I have lost alot of my skills but I have gained alot as well. When you do home health there is no nurse around the corner to give you a second opinion...........I think you should go for it......but know ahead of time you will lose some of your skills but gain others.

It helps me read you. I still dont know which decision I will take but I feel that being AHN of the unit I have been on for the last 12 yrs is great and so difficult all together but most of all, my best nursing is when I support families through the illness of thier child and help them become indepedent and skilled. Seing them leave and know that they can manage is so rewarding.... I need to be in touch with patient!!

I used to work as a general duty nurse in acute medicine x 3 years. At the end of 3 years I was ready to either leave nursing

(couldn't handle the shiftwork and the negative impact on family life) AND felt that something was missing) or get into something with more regular hours.

I lucked out and ended up in an outpatient daycare and loved it!!

Sure you don't get the shift diffs or w/e diffs but you also don't get the shiftwork, work weekends, holidays or the sore back from all the heavy lifting. I learned a lot. If you love patient contact and teaching....there is even more in outpatient clinics, especially if it is a specialty chronic disease population like nephrology.

The patient /family teaching is core to outpatient care and there is more autonomy to practice. Without the docs around as much,

often you are the one to decide that the outpatient needs to become an inpatient and get the ball rolling. I loved the continuity of care; we had primary care nursing/case management.

I resigned 9 years ago to relocate with my family to another country and I still think of that job as one of my favorites.

If I were to return, I could 'never' get into such a plum position again because these jobs are posted internally and are gone in a flash. You are very lucky to be presented with such a great opportunity IMHO.

If anyone wants to know...I will try it but It makes me so sad to leave my gang. Life is changing constantly...hope I am in good challenges

Change is hard for everyone, ask yourself why do I want to leave a job that I love. Can you take a leave of absence from your current job? If so then you could try the other position and know for sure if it is the right decision. Good luck.

I am happy today. Got a job offer for a case manager under the managed medicaid system. I am taking it............wooo hoo....
