One quiz question I am struggling with

Hi,

I am finishing up a OB quiz and am stuck on one question. The question is: A 16-year-old is making her first prenatal visit to the clinic in her fourth month of pregnancy. The nurse's first responsibility would be to:

a. contact the social worker

b. develop a trusting relationship

c. schedule the client for prenatal classes

d. teach the client about proper nutrition

I have ruled out a and c and am trying to decide between b and d. I'm hung up on the phrase FIRST responsibility. It would seem to me that developing trust would be good to help ensure that the teen would follow through with appointments etc, but emphasis on maternal nutrition is also extremely important. It comes down to picking the most correct answer. Any suggestions?

Forget that she is 16 and pregnant. This is a first visit. What is important about that first interview and comunication. I would answer b.

Also without trust, she would not follow up on any nutritional teaching. The trust has to come first and is the most global answer.

I agree and would choose B also.

Me too - B is the answer...that's the FIRST responsibility.

I'm saying "D" and I'll tell you my reasoning. The client is presenting herself for care, so the issue of a patient/client relationship is implied. I'm always thinking about the nursing process and plan of care with these kind of questions. Of all the answer choices that are up there, and they are all nursing interventions, "D" is the one that I feel would take priority because it is a physiological need of both the mother and the baby and it is something that you would do right then and there, face to face at that appointment with the client. It is patient focused. Also, on Maslow's hierarchy of needs, nutrition is right up there after the ABCs are met.

I would guess B. After you develop a trusting relationship, you can teach her the rest of the stuff.;)

I would pick D, simply because a poor diet may put the baby at risks for future medical issues. The baby needs good food for the purpose of development. I maybe wrong, but that is the answer I would pick. Did not take maternity yet.

I think "D" would be the correct answer. Teenage girls diets are notorious for burgers, fries, shakes, or the other extreme, eating next to nothing. The fact that she went to the doctor implies that she is looking for help from an adult so I think trust is not the issue. From now until the end of the pregnancy is time when the fetus needs protein, calcium, and vitamins/minerals to grow.

B. You have to have the trusting relationship to get anywhere with a patient.

my choice would be b because it don't matter if you lecture her until the cows come home, if she don't trust you and what your saying she wont hear a talk on proper nutrition. remember we are dealing with a 16 year old.....

If the pt doesn't trust you, then she won't listen to what you have to say about nutrition anyway.
