GETTING WRITTEN UP: What is the most ridiculous thing you have been written up for?

One night the Don of the LTC place where I worked handed over last month's MAR's and TX sheets and asked me to go through and find the "holes'' and fill them in. I filled out missing blood pressures, weights and accu-checks that I searched through Nurses Notes for and left the rest blank.

I can't believe I got busted for not forging paperwork. :uhoh3:

I never have actually been written up, although I've written myself up for a few minor mistakes. I didn't even get an official write-up for that incident several months ago when the Hemovac tubing broke off inside a patient's knee during discontinuation by the nursing student I was supervising........guess my nurse-manager never got around to it. It was one of those things that was NOBODY'S fault, but the patient did have to undergo a second operation and stay in the hospital longer because of it, and you know some poor fool's head has got to roll when that happens. I still don't know if anything else ever came of it.....but I'm glad that head wasn't mine.

Noticed count on narcotic off by one (AM nurse didn't give). I'm a nice guy but not all that bright/experienced yet. So she's not having any pain and I give next scheduled dose and note the count is off. Got wrote up for it to cover the other nurse (I do believe).

I can't remember if this was actually written up or if the ER doc just threatened to write it up, but anyway...ER calls up to OB, saying there's a gal in labor coming up, "ready to deliver." I asked what the dilation might be (wanting to know just how fast I needed to get the skates on) and was told by the ER doc that he hadn't examined her. I muttered something about not being aware that windows were now availlable and he informed me that he could tell by looking when someone was ready to deliver. She delivered all right; about 6 hours later.

Lord some of these things are ridiculous. I have never been written up...but have done occurence reports on errors I made...so in a way, I guess I have been written up. NO major errors, but reportable none the less.

I've had a few occurence reports as well. People mistake incidence reports for write ups. I love the threats of another department "I'm going to write you up for such and such....." My answer: Sorry, only my manager or higher has the authority to write me up. You as a housekeeper/rt/xray tech/doctor can not write me up. Feel free to talk to the manger". What's funny is when I've had my moments of when I've had to get assertive, stand up for myself or whatever, when I've said to the supervisors "write me up or fire me, but I'm not......." and I never get wrote up. Couldn't get wrote up if I tried. :rotfl:

I got points taken off my eval for not wearing makeup.

An MD screamed at me on the phone and hung on me. I charted the incident and my UD tried to have me fired for it.

LOL! I have never been "written up", but I did have the lab threaten to do so for my bad attitude.... I almost shot them... I had done blood work on a sugar baby 3 times!!! The first time they "just didn't get it", the second time "it broke" and the third time the sugar was 1.4 and they didn't call me (and the accucheck was 2.5)!!! So I called them and we had a "discussion". The woman wanted to know my name and when I asked for hers, she just gasped and hung up!!! I would have laughed if I wasn't so mad.

for working overtime when mandated by supervisor. hn said i should have insisted the supervisor call another staff member to come in. she wanted her "pet" to get all of the extra work so she could have more money for down payment on a house. i went to cno and got it removed. now this hn is in big trouble with same facility. seems that her skills as a manager are finally being questioned. she may lose job when unit is moved to another wing. pay back is sometimes sweet.

i put a band aid on a lady's fingernail because it was snagging her sweater. i didn't have clippers on my med cart but i was going to clip the nail during treatments. unit manager was lurking in some shadow and saw me do it. forgot to get the doctor order for the band aid. :rotfl:

We were just talking about this this weekend! A colleague was written up for not circling a medication on the PRN MAR with parameters (in other words, "give _____ if sbp> or dbp>"). The patient didn't fall into the parameters, so she didn't give the medication. Yes, they wrote her up! :uhoh21:

As an LPN: I had two patients in a room. One had a foley. I had a plastic graduated container in the bathroom and didn't have the patient's name on it.

Same HN mentioned my "body language" on my evaluation. I wasn't giving her the finger so I'm not sure to this day what she meant.

I had a TURP patient whose Foley was removed. Strict I&O. He told me he needed to go to the bathroom. I asked him if he could use the urinal; he replied that he had to do "No. 2." I stood outside the bathroom door to help him back to bed and heard him tinkle in the toilet. I wrote this on the I&O sheet. The next day the doctor had a hissy fit. I explained what happened to dear HN. She looked at me like I was nuts and told me to keep an accurate I&O. That night the same patient peed the bed. I wrote a half-page narrative note explaining why I had not kept accurate I&O on the patient. Never heard a word about it.

We also had a semi-retired RN on that floor. She was a really sweet lady and great with the patients. She had a pre-op patient (back when outpatient surgery hadn't really taken off) with a sleeping pill ordered. She went into the room to give the pill and found the patient sound asleep. She decided that waking someone up to give them a sleeping pill was ludicrous so didn't give it. She got written up. The anesthesologist got that one removed when he said that he thought she showed good nursing judgement.

Lovely head nurse on that job. It almost persuaded me to flip hamburgers for a living.

I was working in a LTC facility and had perfect attendance and no tardies. On my way to work, I hit a deer and of course couldn't make it to work that morning. I did however show up about 4 hours late. (Had to get a wrecker, call state trooper, make sure I WAS OKAY, etc) I called them and told them that I wouldn't be in because of hitting a BIG GIGANTIC DEER that destroyed my car. So, I was written up for being 4 hours late. And no one asked me if I was okay. Later, I was able to get it taken out of my record but after showing the police report, the wrecker bill, pictures of the car with the deer blood and guts all over it and threatening to bring the remains of the dead deer!! My DON said that she never heard of anyone hitting a deer in the road. (I live in a mountain town with curvy roads and home of the deer and the job was in another state in the city.)
