Help! How to proceed? Blood Transfusions; Ddocumentation

I am in an RN internship in an ICU. I'm still on orientation and it is about my 5th week on the floor. Last night me and my preceptor got 3 patients. It was really busy, even more when one patient went to cath lab and came back with a balloon pump. My preceptor was also not feeling well so she tasked to me give all the meds and other treatments that I can, to the best of abilities. One patient needed 2 blood transfusions. All went well. I was able to give meds appropriately, although slow, because I was questioning everything and that annoyed my preceptor. Also I missed other details due to my slowness, which i shouldnt feel bad about since im new.

Anyways, the shift ended I went home, I woke up 2-3 hours before I have to get ready to go to work again, and I realized that I may have forgotten to end the 2nd blood transfusion on the cerner bridge software. It has been over 12Hrs since.

How should I deal with this? I feel like I should talk to the director, I dont want to speak with my preceptor, but I am scared. Is this going to be a cause of job termination? I dont want to get terminated. How would I even go about finding another job if I got fired and have not enough experience... I moved just for this job into a new place. Im so stressed out right now.

Help me figure this out please. thanks

You are on orientation. Orientees are expected to make some mistakes at time. Do not globalize what may be a mistake into termination.

Go to work. Speak to your preceptor and ask her to review the shift with you. If any mistakes are discovered, ask how to correct them.

That's would not be a typical cause for termination. Speak to your preceptor or the charge nurse on duty for further instructions. There is no reason to go above their heads and involve the director ...and why would you want to point out your faults to the director anyway?

I thought of talking to the director or the manager because they seem supportive, kind and they like to teach.

I dont like my preceptor. she just sighs, scoffs, roll eyes, walk out and ignore me for a period of time.

The Unit is really understaffed

When you go back to work if the pt is still there just backchart it. Its not something to worry about terribly. I see it occasionally, there will be a transusion recorded as "transfusing" but it was like 4 days ago.

Everyone knows that the blood is obviously not still running! No one will be alarmed and you def wont lose your job. For me in EPIC, is just a matter of hitting stop in the mar.

No need to go to director! Really, your preceptor should have been checking up that u did all the steps. This is minor in the scheme of things, but its kindof on her!

Just go to your preceptor and get right to the point: "I need to check the EMR to make sure I ended that second transfusion correctly." Make good eye contact and try not to carry yourself as if normal questions are the world's worst sin. It's just one of those things...if you are matter-of-fact and professional, others are more likely to follow suit. And if they don't, that's on them.
