Huh??? You held it for a HIGH blood pressure? I almost want to change the orders to read: "Hold if the top number is less than 120." Can people who went to nursing school and passed the boards really be this stupid????

Is it possible that high schools and colleges no longer teach or that nursing schools no longer require higher level math that use those symbols?

Nope. I have to take a intermediate algebra math class for my BSN completion. Sure to use that symbol. Some just get it mixed up consistently.

Like people who can't for the life of them keep Right and Left straight in their head and always have to hold their hands up with their fingers in an "L" shape and see which side looks like an "L" :)

:wideyed:I've never heard of looking for the fingers that spell "L" when you hold up your hands! What if they are dyslexic and the read their hands backwards? :roflmao:

I could excuse one time but today I had to do an investigation....one woman got the medication 23 times outside the ordered parameters! One guy had 104 chances to get the medication. IF it had been given according to the parameters, he would have received it only 2 times in the month. I told the Nurse Practitioner to discontinue the med. We are not running an acute unit where we have time to take someone's BP every 4 hours.

Is it possible the nurse documented "not given" because 158 didn't need to be treated? I know I have documented "held" when patient had a BP of 161/88 and I didn't give IV BP med that had a parameter of SBP greater than 160.

Nope. We use an EMAR. An x indicates not given. A check mark indicates it was given. The blood pressure is attached to the order so when the order reads: give if BP is greater than 160 and the blood pressure documented is 125, there is a problem.

I do this when I am facing the patient to remember which is my left to know it is their right LOL.

Some people just ignore the details that in their opinion don't matter. How many times I had to explain my doctors the basics {as a patient}. I can't wait I become a nurse, I am pretty sure I will be hated.
