Hi All,
what is the common terms or words we use as a school nurse for student 's visit to the clinic ?
example : burns , scrape , bumps, bruise , slide friction , skinned , so and so....
can name a few ?

All of these plus: all bleeding stops eventually, you have a sub today right? Go back and get a pass. You hurt your (insert body part) 4 days ago over the weekend and want ice now? Go back to class your ok. I don't fix (glasses, shoes, pants).
If the OP meant documentation language then we create our own NDs in our system e.g. Altered skin integrity: laceration minor, hematoma/bruise, etc.; epistaxis, ineffective breathing pattern etc. In the action section we can create drop downs such as washed with soap and water, administered PRN medication etc. In the comments section: Visualization and palpation unremarkable or whatever you see or feel, partial or full ROM, gait brisk and steady, expiratory wheeze bilaterally etc. I have also charted in the notes section when I'm feeling snarky: invisible cut.

Love it! I've been known to chart: Student states he is too ill to stay at school, playing around in clinic, laughing with friends, refuses rest, return to class.

-Kid comes in clinic smiling and has exaggerated limping..."Would you really stop playing at home with friends to go inside for an ice pack? No. If you are smiling when you walk in here, then I think you are probably fine".
-"Your chin, left arm, and stomach hurt? Those are some random complaints."
-"If you don't have a fever and you aren't throwing up, then I cannot make your parents pick you up from school."
-"Your mom said you are fine and you need to go back to class."
-"All three of you are suddenly sick at the same time? Is there a substitute for your class today?"

I'm going to start using "no detectable bleeding" for my little ones who come in and have to first find their injury and them have to squeeze it to show it that "its bleeding" lol
