Would you be willing to do this?

Hello everyone!

I am a new grad FNP. Long story short, I found a doctor who is looking for an NP who can work at his busy Internal Med clinic. However, when I initially met him, he told me that he would prefer me to shadow him/volunteer at the clinic for a month or two. Basically, he is asking me to do "clinical hours" (just like in school) for free. He would like me to observe how he practices medicine and at the same time wants to know if I would be a successful NP at his clinic. He said that he hired NPs/PAs before (without shadowing experience with him) and they all did not do well. He believes that this volunteer experience will benefit both of us.

He did not promise me the job. All he said was that the position is currently available. He alse stated that "Worst case scenario, if i do not work there, i will gain the valuable knowledge and experience by following him." I like the practice. I wish I would be able to work there someday. But at the same time, i would like to hear opinions from NPs regarding this matter. I feel like it's unfair to do clinicals again (after already graduating) without any type of compensation or even verbal promise that I am getting a job offer.

Question is: Would you be willing to do clinicals (again) for 2 months without any promise of getting the job? For free? He requires two to three full clinic days per week.

He's not making you do training for AHA forms unpaid. You're doing it...period!

What's making you think this is going to be more than it already is??

Insurance companies won't pay for your services unless you're signed up with them. That involves a collaborative agreement between you and the doc in most places. To prescribe meds, especially controlled meds, you have to apply, specifying a place of employment. How does the doc plan to get around these requirements? This just sounds sooo fishy to me.

He doesn't expect me to prescribe yet. However, he asked me to undergo training for AHA as a volunteer. This felt like my last straw.

Or, he could have a hole he only needs filled for two months.

I, too, am a new grad. I'd counter offer - three days of shadowing followed by a six week agreement with an opt out for either party at the end. If I really wanted to work there and had had difficulty finding a position.

Otherwise - no.

Sounds like a really great idea! But basing it on our talk today, it seems like he wouldn't want to do any of that :(

Sounds like he is looking for someone to do the 500 AHA forms for free. I've recently made the switch from inpatient to outpatient setting so I'm not even sure what an AHA form is. I do not do a ton of paperwork in my new role. Is this even a form that has to be completed by a provider? How is filling out forms going to show him your clinical skills/knowledge?

Yes, it has to be completed by a provider. It is part of the annual wellness visits that insurance companies require patients to have. From what i know, clinics get paid a lot for these forms. You made a very good point! Helping fill out these forms won't show him any clinical skills or knowledge. I already ran away this morning.... Lol ;-)

No way. I would have be offered the job first. And still no not for 2 months without any pay. I would take a lesser pay rate for a probationary period. Sounds like he's trying to get some free labor

i have considered this as a way to break into a specialty I have limited experience in though.

If he is considering a new grad physician to join his practice, would he ask him to shadow with him for 2 months before hiring him? No, he won't. You are not below him and neither are the NPs or PAs he's hired in the past. His perception of NP role is wrong.

I have worked with a lot of family practice, internal medicine and hematology physicians.

This fellow sounds like a selfish inconsiderate jerk....I dont think you will ultimately be happy working with him.

He will not likely ever pay you well and always be trying to squeeze free labor from you.

I would NOT agree to a 2 month orientation without pay. This physician sounds like an inconsiderate jerk that only values his own time! He likely would not be a great mentor when one is needed (during those first years).
