We just got new ones, they have the name you go by and under that your full name, under that, your title and under that the facility name.
Large name tags and I am not sure I like having my last name on my name tag (have never had one with it on there)
They are pretty darn big.
I have two tags, the hospital issued one with the magnet strip to let me into the building, changerooms etc , which has full name, registered nurse and fab photo on (NOT!) , I keep it clipped inside my pocket or backwards on my uniform. I have a smaller one that has just my first name in large letters, with RN also that I wear for the patients and family to see.
I understand that there have been some problems with nurses getting stalked outside of the workplace, but I have never minded having my last name on a nametag and always thought that it looked tacky when someone covered it up on the tag.
Just my opinion, I know that a lot of people have concerns out there. Frankly, I have an unlisted phone number
I am not a nurse yet but thought I'd chime in. The local hospital here has the first name in very large letters the last name in microscopic letters, their title (RN, PT..whatever) and a picture.
The hospital I had my daughter at has the first name only, title, and picture. I was told that a nurse there had a patient stalk her..serious stalking, calling her at home, driving by her house, parking a little bit from her house sitting in the car, watching her go to and from her car to work etc. He knew a lot about her, all because of her full name on the name tag. She had a restraining order put on him and he was eventually arrested and put in jail for violating the order. When that all went down, management decided it would be better and safer for all staff to only have their first names on the tag, with no last name or last name initial.
I detest having my full name on my name badge for school. I go to clinicals in the area I grew up and have run into people who I'd rather not have them know my "new" (married) name, I don't want them to be able to get my full name and get access to my phone number and address.
Our badges have our first and last names. Picture, hospital logo, and title, and departnent. I work neuro icu and my badge says pt care services. We have a tag that goes under our badge with big bold letters for RN, nurse assistant, MD, etc.
I would rather not have my last name on my badge, but it is there.
My workplace has very generic powder-blue name badges that resemble credit cards. They have the magnetic stripe in the back that we swipe when we clock in and out for our shifts. They have the faint picture of a clock on the front.

Our badges feature our first and last name, and legal titles. The facility used to have badges with picture identification, but stopped doing that about 3 years ago due to the horrendous employee turnover rates. Apparently, it became too much trouble to prepare a name badge with a picture for a newly hired employee when that same worker wouldn't last more than a month or two before quitting.
I am so glad to see this post. Up until now, we had our picture, hospital name and logo, first name, last if we wanted, title, dept, and a bar code with our employee ID #. We are now going to madatory first and last names. All of the new employess are complaining and apparently we all will have to get new badges. I cringe at this becasue we have a state prison in town (and a federal in the process) and we treat the prisoners. It is not uncommon to have at least one prisoner on the floor at all times. I put my name tag in my pocket when I enter their rooms, but sometimes when answering a bell when it is busy, and the patient is not mine, I don't have time to do so before realizing that my pt is shackled to the bed with a correctional officer sitting in the corner. Last names are a bad thing on a badge if you ask me.
My badge has my picture on it, the name of my facility with logo, my full name, title and department. I cover up my last name.....and have two pins with RN and CCRN on them......no one has said anything to me about having my last name covered on my badge and my credentials, I earned those and PROUDLY display them on my badge. My facility has a contract with the local state prison so that's the big reason why I keep my last name covered.

When I was a staff nurse, just first name and degree. Now that I'm an APN, I put my full name, degree, etc.
