Update and HELP!

Okay so a little update from my previous threads, as I thought I was taken off the schedule due to me giving my two week notice, I went to talk to my boss. She and I sat down and talked for a while, and it turns out they wanted me to stay. So they offered me a great schedule, and we also had a meeting about how things were going to change around the agency (all positive ones). So I stayed, and did not proceed with getting the other job. Now it's June, and let me just say, "WHAT DID I DO?". I really thought this agency was going to change, but it has just gotten worse. I don't know the "rules" as far as it goes for recerts and all, but our DON handles the recerts, well some of them. I have been seeing patients who are not recertified, and I was just wondering about that. I guess I don't want to question my boss, but I know that is something I am going to have to get over..as this is my license I am protecting. Another thing, I am being given patients the last day of the week, and there is no way I can go see them, so I call the scheduler and she states it was a mistake on her part, but if I can go see them anyways:banghead:. She made the comment to me that if I don't she will get into trouble???? So she is basically leaving it up to me to get into trouble or not because my boss will see that I did not see the patient..I hope this all makes sense. Some patients have not had their medications refilled because the DON had to get their discharge papers to reorder them, and he has not done that either..I have already told him three times and nothing....I am so confused fellow nurses. Please help! What should I do? P.S. We have an inservice on Monday...should I mention all of this to my boss, or is it better to just move on? I have already thought about quitting, have not found a job yet, but I feel bad because they really wanted me to stay and worked with my schedule...WHAT SHOULD I DO????

My husband was a high-level corporate type for many years. He's always told me to NEVER accept a counter offer given by your employer when you give notice. If it's about $$ and they offer you more, why weren't they paying you more all along since they now think you're worth it?

If it's about schedules or working conditions, why didn't they work with/accommodate you before you felt impelled to look for another job?

I say, get out there & look again. There is no chance with this employer.

LEAVE! Don't let any sweet-talking change your mind, This place is dysfunctional and it's not going to get better. Get out while the getting's good.

Leave! If you want to stay on while you look for a new job, do it because the paycheck still coming in is beneficial to YOU. But come to the realization that this place is not going to change and you have to do what is best for YOU. Here's hoping that you find a new job sooner rather than later!

@ roser13 I never really thought about it like that....thank you! I just feel bad because they worked with my schedule, but now I'm seeing that I really wished I would of just left. You said that your husband stated he said to never take a counter offer in a job...is that for the reasons you stated..or was there something else? Just curious. Again thank you for your response, it really did help.

He said that just because of the reasons I stated. If your employer was willing to pay more, be more flexible, or staff better, whatever the issue is, why wouldn't they do it in the first place in order to provide a safe working environment and cont employees?

That is so true! I seriously thought I was being a bit dramatic, but as I talk to more people, they say the same thing, that this place is not a great place to work, and sadly their employees or patients are not their top priorities. I know that in all workplaces there will be issues, but I know I can find something better, and where I feel safe and content where I work. Thanks again!!

Bottom line, if you are seeing patients who haven't been recertified, you are working without MD orders, and that is illegal. Period.

If your DON is doing Recerts off of a visit done within the 14 day time frame on a regular basis, that's shady ethics at best and Medicare fraud at worst, since the attending MD is supposed to be notified BEFORE the Recert is done. If this doesn't happen and the MD refuses to sign the Recert POC because he/she was not contacted and wanted Home Care stopped, your agency will not be able to bill for any of those visits.

My advice?? Run fast, run far and don't look back.

Yes I am not sure if he really is or isn't seeing the patients, I have heard from some patients that i am the first person who sees them..so that is what really scares me. Thank you for the information, I knew there were rules, but wasn't clear on them. I've heard that the MD that most of these patients have refer them to the agency..so I'm not sure if there is a connection between the two, either way, the DON should be seeing these patients. There have also been times when a patient has been aggressive towards the two nurses before me, and I see they still schedule me to see that patient!!!!:no: . THere are just way too many things going wrong here, that even as a new nurse, I know are wrong. I'm outta this place quick! Tomorrow I finally turn in my two week notice...now to prepare for the awkwardness :/

They sound shady but re the recerts, if an RN has visited in the last 5 days of the cert period to recertify and there in an interim order you can see the patient, ask for a copy of it before making the visit. They can't bill until everything is signed.

Thank you to everyone who responded, I ended up sending my resume to several job posts, and got a call back on two, went on an interview for a surgery center, and got the job!!! Woohoo!! More experience to get. So did end up putting my two weeks notice in, and once again they counter offered with keeping me PRN, and I declined. Next week would have been my last week at this job, but like the last time, they took me off the schedule, this time I'm not going to call and ask what happened...today at work, I had called and texted the DON for a question and he never responded...really?! OH well onto the next chapter in my career.
