I was having this discussion with a friend, but we later agree to to these. Hope this will benefit all nurses student still in school or awaiting to take NCLEX. LIDOCAINE- confusion and drowsiness

BOTULISM is a life threatening situation that need to be care for firstly which can cause brain damage if treatment is delayed.

What will you do,if you notice a pt who is on IV replacement experiencing dyspnea, R 34, and HR 120. You check is lung sounds bilaterally and heard crackles. what would be your best action

1- Notify the physician

2- Perform suctioning

3- decrease the IV flow rate

4- continue to monitor the pt.

Irreversible syndrome-delirium tremens (autonomic hyperactivity characterized by tachycardia, sweating, tremors, htn, dellusions, hallucinations, and extremely agitated. can be fatal if not treated. cardiac monitor, thiamine, Benzodiazapines for sedation, O2 2 liters via nc.)

An 18 years old pt sign and informed consent to have her left knee amputated. but after medicating the pt, the nurse discovered that the surgery is to be done on the right knee. what will you do in other to correct the mistake

1- cross out the LEFT and write in the RIGHT and initial the consent.

2- ask the pt to sign the consent form

3- have the parents to sign the consent form

4- notify the operating room and reschedule the surgery.

1) cath. in the arch of the aorta.

4) I dont like any of these options, but would have to say #4. How about informing the surgeon???

Thyroid storm- very high fever, increased hr, n/v, diarrhea, irregular hr, weakness, heart failure, confusion, muscle weakness. Interventions - LOC, patent airway, resp. status-rate, depth, sounds, O2 sat, anticipate endotracheal intubation and mechanical ventilation, O2 vi nc, frequent rest periods to min. O2 demand, hob up to max airway, heart rate/rythm/sound...lots more

Fruity smelling breath- ketoacidosis, occurs when lungs are blowing off excess acetone. results when insulin levels are too low to transport glucose into cells- diabetes. treatment need to be immediate. Blood gases- low ph (below 7.35), and bicarb is low (below 20??) Not sure about that!!
