On my score report from ATI, it says the Institution is: U of PHX Livermore BSN so I'm assuming I would put University of Phoenix Livermore/Livermore, CA for Place/Location TEAS was Taken. Thank you for your help!
Yeah, I asked the health science department and if you took it via remote proctoring, you can just put the '"university name" REMOTE PROCTORING'

I don’t think they’ll be too picky on this given the circumstances. I was worried about them having my official results because they don’t send a confirmation email once it’s received by ATI, so ----------------------- emailed me back that she has it on file from CSU Stanislaus which is who I took it through, even though it was removed. So now I’m worried, should I have put remote proctoring? I know its the little details we worry about ?

Interesting! I also directly called them but the lady who answered the phone kept telling me she really didn’t know the answers to my questions. I’ll call back next week! They didn’t reply to my email so I’ll have to try something new! Thanks for the help!
Hey all! I received answers on the email! this should help some of you!
“You need to enter all prerequisite courses attempts in the online ADN application.
You should list the English class with the highest grade that is also equivalent to one of the ADN Program English prerequisite courses.
For the recommended courses you should list the equivalent course with the highest grade.
You should enter the prerequisite course information exactly as it appears on your official transcripts.
You should list one AS degree as it appears on your official transcripts.
You should list the location for the ATI TEAS as it appears on your official transcript of the ATI TEAS.”

Hi! 84 points is great! Yes just be sure to list your courses exactly as they appear on your official transcripts from the other school. When you are invited to submit documentation, include course descriptions from the catalog year you took the course and the catalog cover from that catalog year! Wishing you all the best!

Thanks for the info! I have another question though. We don't mark EW as a repeat right? The thing is when I do that, it messes up my science GPA on the last part of my application. But when I mark it as a repeat, then it fixes up my GPA correctly. This is really confusing me.

Right, you don’t list it as a repeat. I called the Health Science division today and spoke to one of the Administrative Assistants on the phone. She told me even though it’s not written down, that I should not list my EW whatsoever and leave it off completely.
Try calling or emailing about it! Let us know if you hear back the same thing I did- to leave it off completely.
It’s also possible they will talk about it on March 15 in the info session.

You’ll talk with them about our program courses, retention rates, the application dos & don'ts, statistics, and any other questions you may have.
This session is virtual. A link for this session access will be provided a week before the event.”
