To reschedule or not to reschedule?
Hi everyone – I took NCLEX for the first time in July, post graduating in May. When I took it I failed, scored near passing†on all the areas and it shut off early. 1 week after my boyfriend of 5 years cheated on me. I was devastated/super depressed for months. Come September I started studying again. Im scheduled to take it for a second time this coming Thursday. First time - I took HURST. When I started studying in September I did the entire Saunders NCLEX Review Board game (800 questions), Saunders Q & A Review Cards for the NCLEX-RN Exam (I did 500 of the 1,000 questions), and have reviewed my HURST study guides. As much practice as I have done, I feel like I am still getting questions wrong, mainly SATA questions – which I did receive a ton of on my first attempt of NCLEX. I am debating JUST studying that 35 page study guide that's floated around allnurses.com over the course of tomorrow and Wednesday and then just taking it Thursday morning. Partly, because I am BURNT out of studying (took an extra year with nursing school after having failed a course..). Then, part of me wants to finish all of those Saunders Flash Cards and reschedule it for Dec 1st (that's the soonest date to reschedule it on). Thoughts? Thank you, in advance

Why is there a faint pulse with severe dehydration? Wouldn't it be thready & increased?
Thank you Lev I understand that HR increases initially, and then decreases overtime once dehydration becomes severe. As for the pulse..I've read (in my NCLEX book..Saunders) that it increases with SEVERE dehydration.. But..this question above (from an iphone NCLEX app; NCLEX Mastery) states that the pulse is in fact faint with severe dehydration..contradicting one another. So I guess what I'm trying to get at is..does pulse increase first (with mild dehydration) and THEN decrease (with severe dehydration)? Or does pulse remain decreased, no matter what the level of dehydration is (mild/moderate/severe)?

Cool limbs Faint pulse Obtunded LOC Capillary refill time of 2 seconds Extreme thirst Bradycardia Pain over lower back Explanation • Obtunded level of consciousness, faint pulse, extreme thirst and cool limbs indicate severe dehydration. • Decreased urine output and capillary refill time of greater than 2 seconds are both indicative of mild dehydration. • Dry mucous membranes and slow skin turgor indicate moderate dehydration. • Bradycardia and back pain are not indicators of severe dehydration.

What is your best advice for answering SATA questions?
SATA: Select All That Apply I took NCLEX in May and did not pass. I'm re-taking it end October. When I first took NCLEX there was SO many SATA questions & prioritization questions. I was wondering if anyone has some magical/enlightening advice that could improve my test-taking abilities with these types of questions. I've done 2,000 questions and have improved tremendously with multiple-choice questions - got that done A+! BUT CANNOT improve with SATA-type questions..! Help! ? Thank you ?

Sorry AlphaM, BSN, MSN, RN, "may have meant well"? Shouldn't he have stood by me no matter what if he loved me? And it sounds like this girl was chasing after him. She's only her 2nd year into college, a not so serious major.., and probably a lot more fun than me... Is he stupid and can't see that I'm a grown women? And love him, just overwhelmed. Im just so confused. He's my best friend. Thank you for being so kind :) It really helps hearing what other nurses/students have to say, hug :)

Thank you ktwlpn - I just feel like I was a mess my last year, it didn't help seeing my friends graduate a year ahead of me and failing that one course hung a lot of stress over my shoulder as semesters dragged on. I shouldn't have complained so much. I think because I love him I felt comfortable complaining around him lol - maybe a little too much. Maybe I just need a stronger man, who can put up with me. Or maybe I just needed to get my act together....

For my last year of school (traditional BSN Nursing; I took 5 years instead of 4 b/c I failed one course my first year of the program), I was complaining a LOT & became emotionally dependent on my boyfriend of 4.5 years. I was in totally denial that I had become this way. Side note: I hate who I am right now & am working on it. Anyways, he told me here and there over the past year not to complain so much - but it didn't penetrate in my dumb head; my mind was focused on school. I love him a lot and I thought he always loved me. He sent me a photo of a place he wanted us to live at one day, cutely, prior to my graduation. And he said he wanted a life with me always. When I graduated he made drinks and appetizers for my parents. Side note: I had a job lined up (for my favorite specialty, might I add) as a RN. I was in such a happy-high during graduation lol. A month later, I failed NCLEX. 1 week after failing, feeling devastated, he asked for a "break" (July 16th) so I could focus on myself and studying for NCLEX again and he could figure out himself. One week later I found out he was "in a relationship" with a random girl. I've wasted the last 4 weeks of my life crying and procrastinating to study. I'm so angry with myself because becoming a RN has been my dream (since I was 15) and this is all I've worked for. But my heart is torn apart. I feel like I ruined us & can't forgive myself.

I plan on doing the following: -All the questions in the "Prioritization, Delegation, and Assignment, 3rd Edition" By Linda A. LaCharity -Questions following each chapter in the "Saunders Comprehensive Review for the NCLEX-RN Examination", 6e book (I'm NOT doing the CD - just book) -All the questions in the Saunders "Strategies for Test Success" book & CD By Silvestri -All the flash cards in the "RNtertainment: The NCLEX® Examination Review Game" board game -NCLEX Mastery Iphone App * Side Note: I really struggled throughout nursing school. I made average of B's/C's. I took the NCLEX once and did not pass. I used HURST review (focuses on content) when I first took NCLEX. I feel this didn't prepare me enough for the actual NCLEX. I struggle with prioritization and SATA questions particularly. Also I did not work as a Nurse's Aid throughout nursing school and I am begging to wonder if this is negatively impacting my test taking skills (not that there is anything I can do about that now..). Also I am putting off applying for jobs as a RN so I can get this down now (living at parent's home)! Anywho..I have all the time in the world to study. I just want to make sure I study efficiently. Open to all comments!:)

Are seclusion/isolation rooms considered a restraint?
The least restrictive method is verbal intervention. The patient should be allowed to stay in public areas if possible, and then moved to isolated spaces. I'm sorry flyersfan88, BSN, RN I didn't mean to be offensive. I just have trouble remembering main concepts (in this instance, the common side effects for a hormonal cancer therapy) if I don't understand the "why" behind it. Also - I can't teach my patients about their treatment side effects if I don't remember them lol. I've heard NCLEX has increased the number of delegating questions!.. but I also think it's important to know the basic content, which helps with delegation

flyersfan88, BSN, RN, if I don't understand how Tamoxifen works then I can't understand the side effects of it. I need to know the side effects for Tamoxifen because it is the primary hormone therapy given to patients with early stages of breast cancer and those with metastatic breast cancer. I also might need to explain to future patients, obviously in simple terms, why they experience these side effects.

AH! Thank you SO much KatieMI ! It makes so much more sense now. So basically, Tamoxifen side effects are r/t stimulation of estrogen outside the breast/uterus. And with the breast and uterus, it binds to a receptor but does not activate the receptor, causing decrease estrogen effects only related to uterus/breast involvement !!!

I am studying for my NCLEX and these side effects are listed and I can't remember these side effects if I have no idea what causes them! I do know that Tamoxifen is an antiestrogen that decreases the effects of estrogen by blocking estrogen in estrogen-sensitive tumors...I'm specifically studying Uterine cancer. And I know that contraception increases clots, but why would an antiestrogen increase clots? Wouldnt it be the opposite then?...... Or maybe because Tamoxifen decreases Estrogen there must be an increase in Progesterone to balance out the decreased Estrogen? And if there is an increase in Progesterone, there's an increase in clots?? So confused... Also I have no idea where those electrolyte imbalances are coming from either...
