I am struggling to think of what would benefit the US healthcare but enrage a certain population. Any suggestions of topics?

making people more accountable for the health problems that they bring on themselves

I'm guessing they're writing a paper about a controversial suggestion for something that would benefit the healthcare system in the US.

I understand healthcare ethics. I've taught ethics in nursing programs. I don't understand the OP's question: "what would benefit the US healthcare but enrage a certain population."

Is the OP referring to a specific "certain population" but being intentionally vague about what group s/he means (in which case it would be really helpful to know which group s/he is talking about), or will an idea that will enrage any individual demographic subgroup do? Would an idea that would enrage more than one "certain population" be acceptable to the OP, or does it have to only tick off one subgroup? Etc., etc., etc. ...

Here is one: Require U.S. Congressmen and U.S. Senators as well as other Gov't workers to buy the same insurance plans as general populace, where as they would have to pay:

* Deductibles

* Co-pays

* Co-insurance

Take the free ride privilege away from them. That in itself will humble public servants and remind them that they are just that, not better and not worse than the rest of us.

Take part of the money that they pay into the system and invest into medical research and health care grants for the needy and disadvantaged

Members of Congress do not get a "free ride" on healthcare. They are in the same federal employee healthcare system as every other federal employee, from the President and his family to people filing files and mopping floors in VA hospitals. They choose which of the multiple plans offered to federal employees best meet their families' needs and budgets. They are already paying premiums, deductibles, co-pays, and co-insurance.
