At our hospital:

Med/Surg - ratio is supposed to be 4:1, but typically is 5:1 or 6:1

Stepdown - ratio is supposed to be 3:1, but typically 4:1 or 5:1

ICU - ratio is always 2:1, sometimes 1:1 if very critical

In the U.S., used to work newborn nursery and we could take up to 8 well babies (in their mom's rooms, but lots of stuff to do!), but 3-6 was more typical (though you often got an admission as the night went on and babies didn't want to wait for day shift! ;)

ED 4:1 or 6:1. With 6:1 we should have a tech, but it doesn't always happen.

Inpatient Adult Psych Unit

5-6 patients, 2-3 if passing meds that shift.

I'm transferring to your hospital!

USA - inpatient psych -

average, 8-9 patients if two nurses on the unit as the scheduling grid dictates...up to 20 if they only have one nurse available (we're hiring but currently short-staffed)... We say the staff ratio is 1 to every 4-5, but that includes the techs, and as awesome as they are, they can't do the meds or daily nursing assessments and flowsheets.

Canada (it depends on the province)

Province: BC

Med-Surg 4-5. (most hospitals have LPN and RN take 10 patients together)

ICU - smaller hospital 2-3, bigger hospital 1-2.

Most places only have one care aid or nursing assistants on the floor of 20-40 patients.

Long term care: the place I was at had one nurse take care of 25-28 patients. For evenings and nights one nurse took care of 53 patients with one care aid/tech/nursing assistant at night.

USA, California to be precise.

By California state law, I can't have more than 6 psychiatric patients in my care. It's not often I have 6 though. I usually have 4 or 5. I had one night not too long ago where I had only 2 patients. I didn't know what to do with myself.

Now, when I wasn't living in California but was still in the US, I have been the sole nurse for up to 16 psych patients.
