Help design new nursing uniform

I tried to complete the survey and it would not accept it when I hit submit. When I attempted to go back and try to re-send the inforamation, it deleted all of my comments. Ah, well. :)

It said that I had errors in lines 9, 10 and 11. I was not allowed to submit the survey. Hmmmmmm....interesting.

This is really bad!

Is this a problem with Netscape or Internet Explorer? It's been a while since I took the survey, and I seem to think that I had to use Explorer to take it, but I can't quite remember.

Please everyone, post the problems you had with it here and I will alert the people conducting the survey about it. Please list what operating system and browser you are using to help them debug the survey.

Same problem - errors in lines 9, 10, 11.

Using IE version 6.0
