To start things off, the best and funniest order I have seen on a chart, was in the discharge instructions for a trauma patient. It read simply
Darwin Consult
and was signed by the resident. Well the attending did laugh, but it was not the highpoint of that residents day.

We have an OB here that writes brief notes on his prenatal sheets--plans, meds, etc. Recently, on a lot of his patients, there is the notation:
"Brief sex--sono next week":sofahider

The funniest H&P-I worked at a large tertiary facility and received lots of patients from Appalacia. The doc down there had lived there all his life (except college). His H&P read, "I was out squirrel hunting with (name of dog), when Gladys came and found me and tole me.........

i was assigned in urology department few years ago in KSA, i was about to go for a lunch break when one of the doctors came in and ask about my patient. the incharge said she will tell the doctor what needs to be done since i handed the patients over to her beforehand. when i came back, as i am reading the new doctor's notes, i laughed with what he have written. he wrote: Patient to continue with NS bladder irrigation. nurse went to the cafeteria when i arrived. didnt assist me when needed. doctors are hungry too! i asked the incharge about what happened, she said the doctor is very upset. he didnt asked for help when the incharge asked him. it was Ramadan season that time and eating isnt allowed when the sun is up with exceptions of non muslim nurses.

Just saw one yesterday - albumin (can't remember the exact amount) NOW an then Q1H x 2. Give each dose of albumin over 2 hours.
I handed the order to the kid's nurse and we just both busted out laughing. Her comment: "Gosh, I know I have good time management skills, but I didn't think they had that much faith in me!"
