Funniest real orders you have seen in a chart?

To start things off, the best and funniest order I have seen on a chart, was in the discharge instructions for a trauma patient. It read simply

Darwin Consult

and was signed by the resident. Well the attending did laugh, but it was not the highpoint of that residents day.

First note that crossed my desk this morning contained:

"Abnormal rythm, Regularly irregular."

Sure it's logical... still funny.

Around here we call that a Jack Daniels drip. WAY better than detoxing the lush!

4oz whiskey on ice at 4p daily. For my no lie 98 year old little lady. Brought it to her every day. If I was running late she'd put her call light on for it. She lived until she was 102 and then some. I still miss her!

The worst one I have ever heard is one my husband told me when he worked in psych @ 25 yrs ago:

GTH 0600 PT - When the confused staff asked what it was, the RMO turned around and said "Gun to head 0600 pull trigger". OMG how could he write something like that in the notes?

I remember myself looking through old notes and noticing that some of the entries were written in rhyme. I spoke to one of my older compatriots who stated that there had been a specialist there who made a point of it...

Go figure...

have seen numerous orders for various kinds of alcohol, always related to ETOH abuse patients, although there was an order in aged care for "one sherry bd" and another for "brandy with warm milk prn"

applying live yoghurt ie with acidopholus and bifidus (not with fruit) to lady parts is great for yeast infection (plus eating yoghurt is good too)
