Frequency: N&T QID?

I have a question about an order for neuro checks for one of the patients on our floor.

The order is for Neuro Checks N&T QID. The "N&T QID" is all one selection for the frequency of the neuro checks, they are not put in separately by the ordering physician. We all know the QID part, it's the N&T part that we are unsure of.

I have utilized my good friend, Google, and have a couple ideas "numbness & tingling" or perhaps "now & today". I am leaning towards now & today because I don't see why numbness and tingling would be part of a "frequency" selection instead of under a "reason" selection. Has anybody else seen this in an order before and knows for sure what it means?

If this is not an approved abbreviation (and it's certainly not one I recognize, but I don't work neuro), the physician needs to be called for clarification.

Clarify the order and get the physician to write the order clearly. :yes:

I agree, always clarify with the ordering MD. It might mean one thing to one physician and something else to another or maybe it was written in error.

Call the MD, clarify the order. "Now and Today" sounds like the title of a soap opera as opposed to an MD order!

Was this a written order? Could it have been transcribed incorrectly? N&T with very, very poor handwriting could be a "neuro checks QID"

Please let us know what the outcome is--I am intrigued.

"Now and today" makes no sense, IMO. Just call and clarify.

No, this was an electronic order so the N&T was selected from a group of options, this was not my patient and unfortunately I didn't get back to asking the nurse if she ever got a clarification, just thought somebody on here might know.

"Neuro-typical"? Meaning both neuro and a "typical" assessment QID?

I've worked with neuro patients for years--1 floor, and 2 SICUs that have included neuro--and I have no idea what that means.
