Dangerous narc delivery by rn

i just started as a 3-11 rn supv @ a LTC that has a new DON, the place is 2 buildings seperated by a gas station, on a busy road, near a bar, group homes, & the occasional street drug deal. the pharmacy would deliver narcs & meds to each building, now the don wants tighter control of narcs, and has pharmacy deliver all to my building, where i log in the narcs, double lock them, but i do not feel safe transporting narcs by myself to the other building, the pharmacy guy also thinks this is stupid and is willing to continue delivering to both buildings, but the DON says no, the other nite i called the male lpn in the other building to come and get his meds and narcs as i did not feel safe, he said ok, but never came by, i left the narcs double locked with the 11-7 rn, a few minutes later, i got a call on my cell phone from the don, yelling at me that i did not complete my duty, that the lpn & 11-7 rn reported me, and i prevented a patient from getting her morphine that the lpn needed, and he wanted me in his office at 8 am the next day (my day off) i cant believe this, what should i do ????

tell him the truth , that you don't fell safe delivering the narcs.... maybe they should have a security guard there ... or someone that could accompany you ... i wouldn't want to do it either....

keep us posted on what happens

they wont supply a security guard, i did hear that they had narcs missing in the other building, so this is the way the new don thinks he can control it better, i did leave a voice mail with him that am, that it was my day off and i had personal things to attend to (also to give him time to cool off, and perhaps seek sane advice) and will meet him thurs. 2 pm

Let him do it. Pharmacy can deliver during the day rather than 3-11. I wouldn't be transferring narcs by myself, period. I certainly wouldn't be transferring them under those circumstances at all.

All that being said, this issue should have been addressed with the DON before it came to a head. A patient did not get her pain medicine that she should have. That's not right either.

Possibly suggest instead of the pharmacy guy bringing you all the narcs that he deliver to both buildings as before and you go to the second building with him to log them in. that way you dont have to transport them alone in the dark but you are there to log them all in.

YOU didnt prevent a patient from getting the morphine...the nurse on the floor did. he knew where they were. if he couldnt get away to come get them the least he could have done was asked you to bring the morphine if he was drowning. from what i understand in your post, he agreed to come get them but never did and didnt let you know a pt was needing the morphine, right?

i suggest when you meet with the DON to not be defensive but be firm when you say you arent comfortable carrying the narcs outside alone. offer solutions.

i feel for you. i, too, have a DON that makes "shoot from the hip" decisions that make no sense. it takes her a while before she will change her mind too.

good luck to you!

HECK NO. Yeah I shouted that. SRSLY, he has to be kidding me. I would think that this would not pass by the state either? I'm taking it that "he" is a your DON? Yeah, maybe he doesn't see a problem in this, but I'm willing to bet that if you ask 100 people, 99 of them will see a problem.

What makes him think that if you deliver them instead of the pharm delivery person, they will not come up missing? Too many hands in the process is what messes things up.

Let us know what comes out of this situation.

Did I miss something?? Why can't the LPN at the second building sign for them? Our driver waits until we account for everything. No way would I go outside with a bunch of narcotics. That is not prudent in my book...especially since the pharmacy is willing to deliver them. Maybe you could say it might violate HIPAA or something. I mean, these drugs have residents names on them right? That DON sounds kinda odd. Good luck. Let us know what happens.

hi everyone, this is a follow up to the dangerous drug

delivery, the don & his assistant called me in today, they were unhappy with my attitude, prior to all this a patient had fallen after 11pm, they wanted me to falsify the accident report to read it happened at 10:50 pm, i asked why they werent asking the 11-7 supervisor to file the report, and why when i came on at 3pm the printer was loaded with lab reports the 7-3 shift wasnt at least screening them, so many things, i had to go to CVS one nite to buy an otc med that the lpn signed out for four hours earlier, but didnt have, a family member complained at 3;10 pm one day about the care her mom got on the day shift, and i passed it on to the social worker and asked the cna to change the patient, the family reported to the Don that i did not examine her moms diaper to see how wet it was, lpn had duragesic patches on top of her med cart, out of the vision of the lpn, we couldnt wash our hands in the facility for 3 days because there were no towels to dry them, too much, and the previous 3-11 rn supervisor is having her license revoked because of a similar situation, she had so much piled on her, she missed a fracture on a patient that had fallen the don and assistant were unhappy with me and asked me to resign and i wouldnt, so they terminated me, they still insist it was my fault that a patient didnt get her morphine and that the lpn that told me he would come get the morphine from me was not at fault, so i feel that a resignation would have been like a guilty plea, sorry that that whole job had to happen, does anyone know who's hiring in the nassau county long island area

How can this be tighter control? YOU have to transport narcs past a gas station, a bar and a group home??!!!! Your DON is negligent.

Just remember, you can't cure stupid....its a terminal disease. Sounds like you are better off far away from that place!

I hate to say it but that sounds like a total setup!!! Think of how many people that would know you are transporting them outside. It would be very easy to do an inside job. If it is a high crime area, some people would have no problem robbing you of the narcs for a low price. You need to run, not walk away from that place! Don't let them put your safety and license on the line!!
