I'm a new nurse in ltc. Yesterday my DON told me to document meds so that it looks like all were passed within the time limit. I'm not comfortable with this. Is this a normal practice in ltc?

Normal, or not, I wouldn't falsify my charting. If they want everything done on time, maybe they should hire more staff.
Especially since we are due for a visit soon. The last couple of surveys this facility has had were dismal. That's why it was sold shortly before I started working there. I have not changed the times on my meds and I won't.

Get another job, when you can. Then report the DON. In the meantime, document the reason the medication administration is outside the window. In this case.. not enough staff is not an allowable reason.
Never hang yourself with false documentation. It is YOUR license.

I wouldn't report the DON if it were me... she will retaliate and find stuff to report about you. I'm sure it wouldn't be too hard to find something you missed or did late or whatever. Quit if you want to but I wouldn't report her for this.

Never EVER falsify documents. Document the correct time given. Find a new job. Any DON that would encourage you to do this is NO GOOD. Don't put it all on the line. Report her!

I worked at a place that asked us to add to our charting and other falsifications. I left there right after that meeting and did not look back.

In all of my years of LTC, we just initialed in the proper little boxes
that the meds were given; we didn't write actual times in... I guess
the OP is doing computer charting.
Not that I ever advocate falsifying times given... I'm just saying,
we didn't write times, just initialed that the meds were given. The
idea of computer charting in LTC is so foreign to me.

Why weren't meds given in the given time? I understand a heavy load, and being called away, but why would your DON actually say document meds in the time limit? HUGEEE red flag to me!

My job recently switched from scheduling meds at specific times to being passed during a general am/pm/hs time frame (with exception of before meals or meds that really need to be at a certain time like sinemet or every__hours. It's been really helpful and widens the time we can pass. I definitely wouldn't falsify charting but you could ask if there are other ways to schedule meds to help address the problem!

I never advise someone to leave a job before finding another....EXCEPT in a situation like this. I personally, at this point in my life, would never show up for another shift there.
