hi to allnurses,it's my first time to post a thread in here but i have been a member for quite sometime. It is such a good help reading forums in here..thank you!

Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..:(

if whatever response i'll recieve comin from the board i'll let you know..about the diploma i think its better you include that together with your declaration and other documents that you can provide.i include mine when i applied at the first time.well,there's nothing wrong anyway if you send it too it might help for them in evaluation of your educational background...

hello guys! im new here and i have been following some post here. I got denied too by the CABRN and applied already for the LVN exam.. Can you send me a copy of the declaration of nursing/license so that i could send it to my school and have them figured out how to answer question number 5. thanks in advance!

I have a QUESTION I received the declaration of nursing education as well just today and was wondering about QEUSTION NUMBER 5? the name of the HOSPITAL(S) that i completed the clinical portion of my nursing education, they provided 2 spaces, is it fine if I only provided 2 hospitals. I was assigned in more than 2 clinical rotations from what i can remember maybe 20 or more. Im so confused because I only provided 2 hospitals that i was assigned in during college, I did put the contact numbers, location, clinical instructor at the time, and dates and hours worked, I guess that should be fine right despite the fact I only provided them with only 2??? or should i provide "ALL" of the clinical rotations on the other attached page? this declaration form is really confusing, unless its just a formality on how honest we can answer their questions since we already provided them with our application form, TOR, and record of nursing program, what more do they want? HOPE someone can help me with NUMBER 5

when we spoke with the evaluator from BVN, she said just put whatever you remember. you dont need to put every single rotation you had when you had your clinical rotation.

You mean to say submit the nursing diploma for item #8 if you don't have a PRC nurse license?

Guys need help! with question #8A - Is it okay if I only send my license? I wasn't able to get a certificate from PRC. Thanks!!!

don't send your license, just a copy though.....

thank you proudpinay..i already sent my form of declaration of nursing but i did not provide the date started and date completed and hours because like what i said i have no list of clinical rotations niether the school.i already sent nursing program form in the school. i dont know what will be their response,maybe im not eligible=(

Hi everyone! I've been browsing this forum since last year but i think i've to start helping our fellow kababayans too. Anyway, as for the declaration form, I asked my school to answer all the questionarres and just have it scanned and emailed to me, in that way all the informations (contact numbers, dates) are accurate and complete. I'll be sending back the declaration form to the BOVN. I will keep you all posted for their reply. :) hopefully i'll be able to get the ATT soon!

hey everyone!. I've been here in CA for about a year now, BSN grad in the PI but haven't passed the local board exam there. Just want to ask, what are the requirements for taking the LVN/LPN exam? :)
