declaration of nursing education and or licensure

hi to allnurses,it's my first time to post a thread in here but i have been a member for quite sometime. It is such a good help reading forums in here..thank you!

Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..:(

I put all the total hours worked in the whole semester which is 408 hours.You can see that on your summary of RLE.And btw,got my eligibility letter 2 weeks ago.

ok, so number 5 questions A and B asking for number of hours worked, you put 408 hours to both A and B? since you got your eligibility. I believe you. btw,congrats :)

applied BON-RN but got denied, I have to retake med-surg and Ob classes , anyone graduated from UC- Cebu here?.. I applied for LVN licensure instead but I recieved this letter of declaration of Nursing Education, has anyone knows hot to fil up number 5?..

to answer question #5. put the hospital were you completed your cases. address. phone #. etc.

about the date started and date completed, we should ask dragon_lady about that :)

Just put the hospital name,director of nursing or the chief nurse,or the clinical instructor,address of the hospital,telephone and fax number, for the date started I just put the day I started my first clinicals of NCM 101 until the last day before semester ends.and for the hours worked,408 hours in all.BTW I did all my NCM 101 clinicals in 1 hospital so I didn't have problem with that,same as the rest.

And I only put 2 hospitals because I asked my evaluator if I can only put 2,so she told me its ok as long as I dont have an internet class because thats the reason why they send me the declaration.

@dragon_lady when did you applied for the NCLEX or is this for the LVN thingy?? how long did you wait for them to tell you, you are eligible in taking the exam?? answer would be very much appreciated ;D

the school must indicate in our RECORD OF NURSING PROGRAM that we have 54 hours of pharmacology, Right?

I sent my application on march 18 and it took 5 months to get my eligibility.
