hi to allnurses,it's my first time to post a thread in here but i have been a member for quite sometime. It is such a good help reading forums in here..thank you!

Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..

One more week and my application would be one month already. No reply yet but I am trying to review even if its kinda distracting thinking about how long should i wait to get a reply from the bvnpt.

Hi, how did you guys fill up question no.5? We had our duties at more than 2 hospitals, so I'll need another form. Where can I get another one? I tried calling the one who sent me the form, but I keep ending on voicemail. I've left a couple of messages, but she hasn't returned any call. Tried emailing her also, but no response too. U just need to attach another paper/s with your other clinicals. No need for a form.

Thanks! I've read here that you only have to put the hospitals you remember. So it's okay if the number of hours you put on the declaration form differs from the nursing education form, right? Oh and did you get your eligibility form already?
