hi to allnurses,it's my first time to post a thread in here but i have been a member for quite sometime. It is such a good help reading forums in here..thank you!
Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..

Can you scan it for all to see? Just delete or black-out any names, reference numbers or any information that reveals things the public doesn't need to see for your safety.
Or maybe provide a link to click to? I think this declaration form comes after a review of your submitted transcripts and application and may not be a regular form on the BVNPT site?

@ posh_888...Thanks for posting kabayan.I havent receive any from the BVNPT yet but I think Ill have one soon.anyways..lets ans.it.:
1)our name
2)our dean at the time of our graduation
3)date when you started college..ex.june 2007-march 2012
4)none if we finished of course from the Phil.and just got here after graduation
5)I think its like the clinical rotation not the clinical cases..so its much easier to produce.
6)PRC is the agency that regulates our nursing practice in the Phil.just check for the address.
7)CHED is the one who oversees our colleges and universities .
8)a.I think photocopy of our license.(if you have one)or college diploma
b.photocopy of our certificate from PRC.(not necessarily if you dont have a license)
9)translator not needed for it is structured in english.

clinical rotation for BVNPT I guess is our hospital affiliations.am i correct?

clarification on #6 - how come when we called BVN they said we should put CHED or TESDA it depends which school you attended, if you attended to a universities/college put CHED but it you attended vocational school put TESDA and they said #7 put same thing as #6.
@shawie...if BVNPT told you what to answer on #6,7 maybe all we need is to follow them.but from what I know it's really PRC that oversee our practice as a a nurse.I think it's for those who are license in our country(RN) because if you looked at the end part of the question #6 it says nursing education license and in #7 nursing education only.
i guess so...
@proudpinay: that's what I'm thinking too write prc on # 6 and ched on number #7. I emailed them, we will see it later on. I might try to call them too monday
