My first LTC job after I separated from service was on 11-7 at a 120 bed facility divided into 3 wings with a total of 5 aides for the whole building....and me. This was back in the days of bulk bottle meds for controlled drugs. I remember narc count literally took almost 45 minutes because of the need to pour out all those pesky little phenobarbs and tylenol #3s, etc into the count trays. I would have killed.....killed I tell ya lol....to only have 19 patients on 1 wing with a fellow nurse & 2 aides. Yep I be a crusty and rusty old bat glaring at the youngens whining.
OMG...19 patients on a short term floor. NO IVs, NO trachs, NO Gtubes.., no sick people.mostly ortho.Nurses on 11-7 whining because they were told they were going to have ONE nurse. Really??? you can't manage 19 stable residents on 11-7? You have 2 CNAs...really? I don't get it.
I wish I had only 19! And if they weren't total care? I have 50 and almost all total cares! 19 would be a dream come true.
Our nurses have 60 patients per nurse with 3 aides on nights. approx 15 would be skilled, all should be relatively stable with the possibility of 1 or 2 issues as far as crashing. That, to me, is too much- but it is what it is... I'd kill to have 30 per nurse on nights, 20 on days, but such it is..
I recently started as don at a small rural private owned facility. Our numbers are:
37 patients, 5 of those are skilled.
One catheter
One colostomy
No trachs
No suctioning
No feeding tubes
3 pressure sores
One bed bound resident
Staffing
M-F
LVN 8a-5p
LVN 7a-7p
Med aide 1p-9p
LVN 7p-7a
Shower aide 8a-5p
Restorative 7am-3:30 p
3 cnas 7a-7p
Hospitality aide 8a- 5p
3 cnas 7p-7a
Saturday
4 cnas 7a-7p
3 cnas 7p-7a
Sunday
3 cnas 7p-7a
3 cnas 7a-7p
Weekend nurses
LVN 7a-7p
LVN 7p-7a
LVN 7a-11p
To me we seem over staffed, which is great, but with this level of staffing, should I still hear moaning and groaning about being understaffed and still be getting excuses for why things aren't done?
