What time is PM and HS med pass typically?

I have worked in a LTC facility for the past three months, not exactly sure how I have made it this far without asking the question but what time is PM and HS med pass typically? Here is my routine, I work 2p-10p I get there, get report and count the narcotics, usually done by 2:20 OR 2:30. Then I prepare my vital sheets for my CNA's and take a few minutes to breathe before the day gets crazy. I have 26 residents most of whom all go to bed right after dinner. I start my PM med pass at 3pm, I have a few residents who receive multiple doses of the same meds at PM and HS so I do theirs first. I get my bloodsugar checks done starting at 4pm (only have 4 of those) continue passing a few meds of people who I know will be in their room. I then get my diabetics their insulin and pills by about 4:30 then head off to the dining room. Once I am there I try to pass all pills that I can PM's and HS's as I said most of them go directly to bed so technically that would be their HS. If I know someone stays up past 6pm I won't give them their HS meds then. I then start my HS med pass at 7p and usually get done by about 8:30p sometimes a little earlier I just wonder if I will get into trouble from state doing this. I am a new grad nurse and this is my first job, any advice out there so as to not get tagged? Also in training I was told (unreliably) that PM med pass was 4p so an hour before and an hour after, so I have from 3p to 5p to get it done, NEVER going to happen! lol Then I heard a couple weeks ago PM med pass goes from 4p to 7p or something.

Your day sounds similar to mine. You should check with your facility as the med times. My facility has a flex med pass so the AM med pass is 6am-10 am and the noon med pass is 11am-1pm

for evening the pass is 4pm-6pm and HS is 7pm-9pm.

That is wha the policy is, now in reality my med pass starts around 3pm and goes untill 7pm when I stop for lunch and then I keep going untill I am done, up to 11pm

In the hospital, BIDs go at 2000, TIDs go at 2100, and QHS is 2200. I'd say check with your charge, or casually ask someone else. You know (unfortunately) if you ask the wrong person, it'll be all over the place that you didn't know when to give your meds, rather than, 'good job double checking something you weren't sure of'.

i work am shift but as far as i am aware...pm are at 4 and 5 and 6 and hs are at 8 and 10.

We have a 5pm pass and an 9pm med pass. The 9pm med pass is for the hs meds.

check your policy. If you have residents that go to bed before the last med pass..you might need to make an order that states may give hs meds at 7pm due to resident going to bed early.

I do what you do, mamarhonda. Except I have 35 people and get done at 2130.

When I work the 3-11pm shift, this is what I do:

3:30-4:30pm I'm doing blood sugars and passing the 4pm meds (usually Synthroid).

4:30-5:30-ish pm I'm beginning my PM med run (some meds scheduled for 1700, others scheduled for 1800).

After that med run, I go on my 30 minute lunch break, when I come back I finish up any other meds I didn't get passed during the first med run. (sometimes I can't find the resident, they went to dinner with their spouse on another floor, etc.)

I also chart before 7 pm if I need to.

7pm I start my HS med run (HS in our facility is usually 2000). Some meds are scheduled after that for 2100, 2200, etc. I pass those when scheduled and chart whatever I need to chart. In between the med passes I'm doing bandages before the Res. falls asleep.

We stopped using HS, we put the time, so if resident goes to bed at 6 pm, med is set for 6 pm if HS med, and our med passes are 0600,0800 1200, 4 pm 8 pm, with many variations on that theme. Again, remember, nursing sets the times, times are changeable, and flexible.

This is set by policy in each facility. There might be exceptions depending upon the individual's difference in habits. In that case, their sheet in the MAR will have their set times for these meds.
