We use preemie XS for up to (P-XS) Swaddlers from Pampers that fit up to 4lbs. They are about the same size of a maxi pad.
Yes, Pampers makes a preemie xtra small. You need to tell your supply people to order them. The big diapers are not developmentally appropriate for the less than 1 kg crowd.Can cause hip problems and such later in life.

Hello all. My unit (level III medical, mostly dealing in premies) uses Pampers diapers in sizes P-S and N (and occasionally 1 for the chronic kids). The P-S diapers look fine on the feeder-grower crowd, but I hate to see them on the very little babies. We have to roll up the fronts so that the diaper doesn't reach the armpits, creating a lump that the babies have to lie on if they're prone. Worse, there's nothing to do about the width of the diaper at the crotch, so the tiny babies' legs are forced apart. It looks very uncomfortable to me, and I doubt it's very good developmental care either.
What I'd like to see are diapers about half the size of Pampers P-S, that would fit comfortably and look sharp and dignified. Do these exist?
