While it can be tempting to want to teach some folks a lesson by making their treatment as painful as possible, this is not considered ethical practice, and allnurses.com does not endorse its use as an educational technique.
Besides the above practice being unethical, we don't always know what's going on behind the scenes. It's easy to judge with only part of the information. Even if we DO know what's going on, it isn't our place to administer punishment . . . . no matter how much it might seem warranted.

Ok, maybe I should've not tried to make a joke on something like that.
The only times I have ever not mixed Rocephin with Lidocaine were 1) when I didn't know you could; and 2) when we were out of Lidocaine.
Sorry.

that's right.
it is not the place of a nurse to administer punishment.
it is not the place of a nurse to be unethical when treating any patient (isn't that the same as abuse?)
I think we're all pretty clear about the ethics involved in this kind of a situation. It's only human to want to "educate" a few patients who seem like they need to wise up. But 99.9% of us would grit our teeth and resist the urge.
I mainly posted the disclaimer so that students and newbies and non-nursing folks don't get the idea that we're okay with "needling" any patients we find to be irritating.

[color=#483d8b]apparently someone once had a reaction (or so the story goes). so now they all must suffer.

Critter, can you mix with lido with a doc's order? If I knew someone was coming at me with a non-lido'd Rocephin, I'd refuse it.

I miss the days when we would give the 4.8 million units of Pennicillin deep IM. Loved the look of terror in their eyes when you walked in with that huge syringe and huge needle To give it to them. Personally I think we are too nice about it nowadays.
I once had to give a very nice little old lady, who weighed maybe 100# soaking wet, a dose of that nasty penicillin. She did NOT have an STD. It was for some other infection. It came up unit dose from the pharmacy w/that 14g 4" needle. I refused. When I showed the intern the needle and syringe, he promptly ordered something else. I have NO idea where I would have put that thing if I'd had to give it--any muscle I could find on her would only have accomodated a 1" needle.
It came with a 14 g needle???? Dang!!!!
The prefilled tubexes of Bicillin----not sure what gauge those needles are but I swear the tips were dulled at the factory. I will not use those needles. I transfer the Bicillin to another syringe and use a 21g disposable needle. I've found the same problem with the Lovenox prefills, the needles seem dull.

I loved those 14 g needles, so inspiring.
Lovenox refills use a specifically designed needle, works to their advantage
I got Bicillin on Thanksgiving Day once when I had strep. The 14g needle injection wasn't a picnic, but the lump that stayed in my backside for 10 days afterward was by far worse. Hurt like the devil.
I have given more Bicillin for various things than I care to count. It is like injecting Elmer's glue. I hate the needle it comes with too and have many times transferred it into something different. The worst for me is giving it to kids!!!
I was tempted many times to do non-lido Rocephin with someone coming in for like the 4th or 5th time with the same complaint after sleeping with a prostitute unprotected. But like rn/writer said, I did have compassion and mix it like I'm s'posed to. And did a little education afterward.
