Well it's that time of year again....July. Those of you who work in a teaching hospital know that the new MD interns start this month. Ours started yesterday...already have had some good stories and crazy orders happen. So in the interest of a little humor, let's hear some experiences with teaching/guiding the new residents.
I had an intern order a fluid challenge on a 4 month old....no biggie. He wanted me to give the child 10 ml Pedialyte every 5 minutes for 30 minutes. "Shall I carry the baby with me while working the head trauma that just rolled into my trauma room?":uhoh3:

STAT Orthostatic BP's (lying, sitting, standing) on a 340lb paralyzed pt with a vent.
I asked the poor guy if he was trying to be cute...

"Dilaudid 2mg IV" - on a 90 lb, 95 year old patient who was 80/50.

"kcl 40 meq iv push" on a 98 year old lady with a k+ of 3.8, no ectopy and a 22 g. in her hand.
"two bananas with every meal" on an npo patient with a k+ of 2.1, runs of v-tach and a big honkin' cordis hanging out of his neck!
ok -- i admit it -- the second order came from an intensivist!

Probably the same that I learned in nursing school - not much. I learned the real stuff on the job.

My favorite is when you call about something and they ask what do you think? I say I don't think anything about the guy who says 8/10 pain and walks around with his PCA pump and half zoned out. :) It is going to be interesting days ahead of us! :)

I taught ACLS to the new interns this week - golly they look so young. Of course, it doesn't help that they are younger than my oldest son! Ugh - I'm ancient.

I know what you mean, some of these new young pups coming to the ER. I have to break them in. I have broken in so many after all these years.
