Many hospitals are moving towards creating mandatory influenza vaccination policies as a condition of employment. Such mandates are causing surefire responses on both sides of the fence. What is the push behind the movement, and what is causing the pushback?
Updated:
First, let me just go over some of the symptoms of the flu, and then we will get down to business on the controversy surrounding the push to create mandatory flu vaccines for healthcare workers:
I can't think of anyone who wants any of these symptoms, or to come down with the flu, can you?
Influenza vaccination research has clearly documented the benefits of receiving the flu vaccine (Google it - you will find a plethora of information...however, I will list some links at the end of this article for your information). However, even though there is sufficient evidence to prove the benefits of receiving the flu vaccine, vaccination rates among healthcare workers are pitifully low. In 2013, only 55% of nurses in the frontlines were vaccinated.
Organizations such as The Joint Commission, the American Nurses Association, the American Academy of Family Physicians, the American Academy of Pediatrics, American College of Physicians, Infectious Diseases Society of America, the Association for Professionals in Infection Control and Epidemiology, the Centers for Medicare & Medicaid Services, the Centers for Disease Control, Healthy People 2020, etc., etc., have stated their position on recommending the flu vaccine for healthcare workers in order to decrease the risk of exposure and reduce deaths. Due to this recommendation, many healthcare facilities are now creating mandatory flu vaccine policies as a condition of employment.
Employers who create mandatory policies will have exemptions, of course. Exemptions are made for medical and religious exceptions. For places that do not have a mandatory flu vaccine, they may "strongly recommend" the vaccination and may have a declination form for employees to submit if they refuse the vaccine. Additionally, some employers will enforce refusers to wear a mask during flu season while they are at work in order to protect the patients and the employee.
Evidence has shown that there are more than 36,000 deaths in the US each year related to influenza, and more than 200,000 hospitalizations. Influenza is the 6th leading cause of death. Healthcare workers are the leading cause of influenza outbreaks in the healthcare system. up to 50% of people who are infected by the flu virus do not fill ill for several days and can spread the virus to people at risk of complications and death from the flu. Additionally, evidence shows vaccination decreases mortality by 40%, decreases the spread of nosocomial infections by 43%, and decreases absenteeism by 20-30%.
Additionally, there is the ethics to consider. As healthcare workers, we have all taken an oath to "do no harm". As a nurse caring for patients who are not in their most physically healthy state, do we take the vaccination in order to prevent spreading the flu to our vulnerable patients, in order to "do no harm"? We must consider this when we make our decision to take or refuse the vaccination.
I have the names of a few hospitals, and this is by no means a comprehensive list. This is based on a ListServe survey of hospitals and these are the responses received:
Refusal may be largely due to misconceptions related to the vaccine. Fears that the immune system will cause them to get the flu, beliefs that hygiene and better nutrition are more helpful than the vaccine, fear of needles, beliefs that the vaccine does not work, and fear of side effects. Others believe that they have a constitutional right to refuse the vaccine and that mandatory policies are violating these rights.
(taken from the Los Angeles County Department of Public Health outline of flu vaccine talking points for managers)
The flu shot does not give people the flu. It uses inactivated ("dead") virus. People may still catch a cold or other virus that the vaccine is not designed match.
Studies have shown that flu vaccination prevents flu in 70% to 90% of healthy adults younger than 65 years old.
You need a new vaccine every year - the virus changes over time.
Serious adverse reactions are very rare. They are explained on the CDC's Vaccine Information Statement, which is distributed when the vaccine is administered.
Local short-term reactions - such soreness at the vaccination site, slight fever, achy feeling - may occur but usually do not last long. Over the counter medicines are helpful. Even short-term reactions are much less bothersome than catching the flu and feeling very sick for days.
Your patients are at-risk, and possibly some friends and family members. You can be infected with the flu virus but not feel ill - and can still transmit flu to at-risk patients.
Inactivated influenza vaccine is effective in preventing transmission and reducing complications of the flu. In years when there is a close match between the vaccine and circulating virus strains, the vaccine prevents illness among approximately 70%--90% of healthy adults under 65 years of age. Vaccinating healthy adults also has been proven to lead to decreased work absenteeism and use of health-care resources, including use of antibiotics. Strong protection is also expected when the vaccine is not a close match with circulating strains, with 50%--77% effectiveness in these instances. In addition, effectiveness against influenza-related hospitalization for healthy adults from inactivated vaccine is estimated at 90%.
All healthcare facilities will be facing the choice of creating a mandatory influenza vaccination in the near future, if they have not already. In order to make an informed decision on the topic, we must have information. Knowledge is power. Before you make a blanket statement on pros or cons, have the information you need, know the research, and make an educated decision.

Big Pharma vaccine profits conspiracy - Skeptical Raptor
QuoteBut the Big Pharma vaccine profits conspiracy is still one of most amusing myths of the antivaccination world.
Nurses like you always baffle me. How can one work in a field that requires so much use of common sense and so much scientific education and understand and yet clearly still not use science or common sense when approaching whatever the conspiracy of the day is? I have yet to meet a nurse that's a young earth creationist but this is along the same lines.
I have taken the flu vaccine twice, and never again. My entire arm turns red, and swells up. I have the same reaction to TB tests, and have been told never to have a TB test or flu shot again, apparently I am highly allergic to something in the preservatives. I am too old to have intranasal. I have been told that anaphylaxis is a distinct possibility at any time.
This is rare. Obviously we don't want someone who has a reaction like this to go against their physician's recommendation.
But again, this is rare.
I, and long proven medical science, disagrees with the statement that just because someone eats junk food and doesn't exercise, it has nothing to do with the Flu shot. It's long scientifically proven that maintaining a healthy lifestyle is the #1 best way to keep a healthy immune response and prevent disease. A pill for this and a shot for that has not had the same efficacy. Primary Prevention and Health Maintenance are the cornerstones of Medical practice.
Just because people get the annual flu shot does not mean they're not spreading the Flu to patients. This year our staff had an influenza outbreak....All who had the Flu are the religious vaccinators. When they all tested positive for the Flu they were baffled and asked why they caught the Flu when they've been vaccinated. The athletic, healthy staff did not even so much as have a sniffle. Why is that? Lifestyle is my answer. And this happens every year where the vaccinated staff have an outbreak. I have witnessed this over more than 20 years in healthcare at various facilities that it's become very predictable.
If we as healthcare providers are TRULY SERIOUS about preventing the spread of the Flu to our patients, then we MUST institute MANDATORY LIFESTYLE POLICIES- i.e. proper nutrition, no smoking, regular exercise, healthy weight. I say all HCPs must be annually weighed in & remain a Healthy BMI, pass a physical exam (iE able to run at least an 11 minute mile), Cholesterol Screening, etc in order to keep their jobs. If the Flu Vaccination of QUESTIONABLE efficacy is REQUIRED, then a Healthy Lifestyle of PROVEN efficacy should be all the more requirement of employment. That is, if we want to walk our same talk about Disease Prevention. Lifestyle has long been proven to be the most effective way to prevent disease and have a Healthy immune response, there's no question about that.
I Refuse to get the flu shot!
I work in the ER and have the highest expose to everything, however
8 yrs in a row I had the flu shot each time I became sick for 2 weeks 3 days after the shot... it doesn't take a genius to find the association. It is not anecdotal
- I refuse to be a test Subject for the pharaceutical industry without extra pay. Last 3 yrs I have refused the shot and have been off only twice once with pink eye and once with hand foot&mouth
- as an RN I have more trouble claiming sick days than anyone.. my employer will not cover flu sickness at all bc the shot is supposed to be perfect.
- if I get the shot I get sick and miss work and $
- who does getting the flu shot protect? The employer who can use it as a loophole to not pay sick time
I agree. I have never had the vaccination (due to copious allergies and autoimmune disease), have never had the Flu, and I do Triage quite often and am exposed to sick patients, AND I work with the religious flu vaccinators who always come to work sick, yet I never catch any of the illnesses. 2 sick calls in over 20 years for me (1 was due to an accident, the other foodborne illness 17 years ago). I'm a firm practicer of primary prevention and healthy lifestyle in my own life. The Flu Vaccination seems like a money grab or bunk science to me...and I am NOT Anti-vax. The Flu vax is junk science. If hospitals are truly serious about COMMUNICABLE DISEASES, then why are they not requiring the DTAP vaccination, given the Pertussis and Diptheria outbreaks? Something stinks unethical when you consider that.
The logic behind this post is spot on. How can something oh so dangerous simply disappear each spring? Im not an anti-vaxxer either but this mandatory flu vaccine is not based on facts. If only 55% of nurses are getting vaccinated why arent we seeing higher rates of infection in the patient population? I had never received the flu shot until i began nursing school. I also have never had the flu. My brother and I (we were kids) lived in the same house with my mother when she had the flu and neither of us contracted it. I have a friend who every single year she's gotten the flu vaccine has gotten the flu: confirmed by the hospital letting her know the strain as well. I don't think the vaccine caused it but it certainly did not offer her any so called relative protection either. The past two years that I have taken the flu shot at work the CDC confirmed it not a good match for the most prevalent strains. I believe there must be something else behind this push to make the flu vaccine mandatory.
QuoteThat's one of the myths I encounter so in order to be honest I have to say I disagree with you.
First of all:
Most vaccines were created before Big Pharma got involved.
Second: You obviously have no basic understanding of economics.
VOLUME is the key to making a fortune.
Big Pharma vaccinates 80% of American citizens with the government picking up most of the tab.
Since most legend vaccines are administered at cost or just slightly higher, the majority of the money spent goes to Big Pharma. So, if $800 is spent on vaccines (I threw out the pneumococcal, HPV, varicella, and flu vaccines because they are useless and likely cause more problems than they solve) and there are 4,000,000 people vaccinated each year, then that is $3.2 Billion revenue with likely $2.3 Billion in profit. That is every year in perpetuity with absolutely ZERO risk. That is just the USA, this does not include the rest of the world.
Now for the FLU Vaccine. Cost to manufacture $2.00. CDC cost per dose $7.69, private party $10.00.
At least $5 profit on each dose for the 125 million Americans vaccinated each year for a total of a bare minimum $725 million profit each year in perpetuity.
Again, this does not include the rest of the planet.
Ummm....Can you guys start your own flu conspiracy thread, please? You're getting way off topic.
No, this is on topic and exactly what the Op expected. This reaction is not a confounding variable, it is assumed that when posting material in a way that slants toward one side of a controversial topic, that the proponents of the other side of the issue will respond.
