I was having this discussion with a friend, but we later agree to to these. Hope this will benefit all nurses student still in school or awaiting to take NCLEX. two questions....first, why wouldn't a client with prolapsed cord be seen first? isn't that considered an emergency? and two, for the last question, how can you press the cannister b4 placing it in ur mouth??

a pt with prolapse umbilical cord has a greatest risk during labor and delivery but not at risk during postpartum period

why pt with prolong rupture is the most @ greatest risk for postpartum infection which make the pt a first candidate to assess.

to effectively use a mdi with a spacer, the nurse will instruct the pt to first shake the canister to mix the medication evenly,

then press the canister to release the medication into the holding chamber,

followed by placing the mouthpiece of the spacer in the mouth and inhale,

lastly the pt should hold his breath for 2 to 3 mins and exhale

Ohhh ok....see the questions weren't specific, so I could not gauge that the scenarios were in the postpartum period, and I couldn't tell that the MDI was with a spacer because it didn't specify that in the question. Also, I have asthma...never have I pressed the medication first, then put the spacer in my mouth, and I wasn't taught that in nursing school, either....I just looked up the technique in my Taylor's skills book just to make sure I hadn't been doing it wrong all these years, and it says to press the cannister after the spacer has been placed in the mouth. I thank you so much for your questions because they are helping a lot, and I am sorry if I seem like I am complaining, but I just want to be clear about stuff in case I do see it on the NCLEX.

Heparin is best given in high doses follow by smaller daily doses. Because the pt in the question is experiencing internal bleeding and heparin should be given quickly to increase the blood coagulation and later can be given in smaller doses daily.:up:

this dosing is right but I would question this order (uness they have DIC). Heparin is an anticoagulant, uncontrolled bleeding is a contraindication. hth! This has been an awesome thread!!!
